Job description

Inspiring Behavioral Services, an affiliate of Preferred Healthcare Group LLC, a Houston based company, is looking for a Case manager/QMHP to enhance the quality of patient management, maximize satisfaction and promote cost effectiveness. The successful candidate will be accountable for the full patient care cycle. Case manager's duties include assessing, planning, implementing, monitoring and evaluating actions required to meet the client’s health and human services needed. A certified case manager is a plus. Spanish speaking is a plus as well.

Responsibilities

Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered
act in a professional and courteous manner always while performing the duties of in-home counselor
Provide primary service in client's home and render all services as identified on the client's individualized service plans
Handle case assignments, draft service plans, review case progress and determine case closure
Help clients achieve wellness and autonomy by assisting clients on any goals as directly related to the client's individualized service plan
Work with clients and family members to foster the development of new improved coping, case management, problem solving and communication skills, etc.
Help clients make informed decisions by acting as their advocate regarding their clinical status and treatment options
Develop effective working relations and cooperate with the clinical team throughout the entire case management process
Submit daily clinical progress notes by mandated deadline
Take the extra mile and interact with patients to keep track of their progress and to ensure satisfaction
Record case information, complete accurately all necessary forms and produce statistical reports
Promote quality and cost-effective interventions and outcomes
Assess and address motivational and psychosocial issues
Adhere to professional standards as outlined by protocols, rules and regulations as is enshrined in the IBS employee handbook

Skills

Proven working experience in case management, including Social work, Psychology, and mental health care management or a related job
Excellent knowledge of case management principles.
Previous experience with psychological aspects of care
Effective communication skills
Excellent organizational and time management skills
Familiarity with professional and technical emerging knowledge
Problem solving skills and ability to multi-task
Compassionate with teamwork skills
Certification or registration of CANS and ANSA is a requirement
minimum of BS degree in related healthcare field

Job Types: Full-time, Part-time, Contract

Pay: $18.00 - $25.00 per hour
