Full Job Description

About us

Sentry Home Health Care is seeking a full-time Registered Nurse (RN) to serve as a Skilled Nursing Provider and Quality Assurance Officer. This position is responsible for delivering skilled nursing care to patients in the home setting while also supporting agency quality assurance, compliance, and documentation review activities.

The ideal candidate will demonstrate professionalism, strong clinical judgment, excellent documentation practices, and the ability to work both independently and collaboratively with the interdisciplinary team.

Essential Duties and Responsibilities

Provide skilled nursing services to home health patients.
Complete Start of Care (SOC) / admissions.
Complete Resumption of Care (ROC) visits.
Perform routine skilled nursing treatment visits.
Perform supervisory visits as required.
Complete 60-day recertifications.
Complete discipline discharges and agency discharges.
Assist with quality assurance and compliance documentation review.
Assist with monitoring clinical compliance, including physician orders, plan of care, visit frequency, missed visits, infection reports, incidents, falls, medication safety, and required follow-up.
Assist with managing and coordinating clinical disciplines to ensure services are provided according to the physician-ordered plan of care.
Communicate with therapy disciplines, aides, physicians, caregivers, and office staff regarding patient needs, changes in condition, and care coordination.
Participate in QAPI activities, including chart audits, infection control tracking, incident review, missed visit review, hospitalization/transfer review, wound monitoring, and corrective action follow-up.
Identify areas of concern and report compliance or quality issues to the Administrator, Clinical Manager, or Compliance Officer.
Participate in weekly to bi-monthly case conferences.
Maintain timely, accurate, and complete clinical documentation.
Maintain professional conduct at all times.

Productivity and Schedule Expectations

Full-time position with a 40-hour work week expectation
Expected productivity of approximately 25 patient visits per week
Remaining work time dedicated to quality assurance, compliance, and documentation review
Standard treatment visits typically average 45 minutes
SOC and ROC visits may require additional time
Documentation must be completed and submitted on time each week

Qualifications

Current and active Registered Nurse (RN) license
Strong clinical and documentation skills
Ability to manage patient care in a home health setting
Knowledge of compliance and quality standards preferred
Home health experience preferred
Ability to work professionally with patients, families, and team members
