Full Job Description

CC Young, a vibrant senior living community and a non-profit leader in senior living and Health Care, is seeking a Director of Clinical Services for our Hospice Department. In this role, you will lead the nursing staff and field clinicians, oversee daily clinical operations, ensure the delivery of high-quality patient care, while maintaining strict compliance with state, federal, and Medicare regulations.

The ideal candidate is a collaborative leader, an expert in clinical documentation, a passionate advocate for patient and their families, and someone who excels at mentoring clinical staff and navigating complex case management in a fast-paced environment.

Schedule: Monday – Friday 8:30 AM- 5:00 PM. Occasional on-call nights and weekends required.

Job duties include, but are not limited to:

Supervise and support skilled nursing staff and field clinicians.
Clinical leadership and oversight
QAPI and quality improvement
Survey readiness
Clinical budgeting and productivity
Staffing and workforce planning
Hiring, coaching, and performance management
Infection prevention and patient safety
Census growth support
Clinical education and competency validation
Physician collaboration
Risk management
Patient and family satisfaction
Community and referral partner collaboration
Operational planning and strategic initiatives
Ensure timely physician orders, care plans, and documentation completion.
Conduct routine chart audits for Medicare and agency compliance.
Coordinate patient care with physicians, facilities, and caregivers.
Provide field support and occasional supervisory visits as needed.
Participate in case conferences and interdisciplinary meetings.
Manage incident reporting and follow-up investigations.
Support orientation and mentoring of new staff members.
Ensure compliance with agency policies and Conditions of Participation.
Assist with on-call support and operational workflow management.
Perform other duties as assigned.

Minimum qualifications include:

Associate or BS in Nursing.
Valid Registered Nurse (RN) license in good standing with the state of TX.
Valid Texas Driver's License and reliable transportation.
2+ years of supervisor or leadership experience as a RN in a clinical setting.
2+ years of hospice experience in the field or management.
Previous Inpatient Hospice Unit experience is strongly preferred.
