Full Job Description

This is not your typical CNA position.

At DayBreak, our CNAs spend their days building meaningful relationships, leading activities, supporting participants living with dementia, assisting with basic health needs, and helping caregivers navigate one of life's most challenging journeys.

With a staff-to-participant ratio that never exceeds 1:6, our team has the opportunity to provide individualized support and truly get to know each participant and family. We are seeking a compassionate, dependable, energetic bilingual (English/Spanish) CNA who is passionate about serving older adults, supporting caregivers, and helping DayBreak expand access to dementia-informed services throughout our community.

Required Qualifications

Current Utah CNA certification.
Fluent in both English and Spanish, with the ability to communicate effectively with participants, caregivers and community partners.
Current BLS/CPR certification.
Current Food Handler's Permit.
Ability to pass a background check.
Must be at least 18 years old.
Reliable, dependable, and punctual.
Ability to work scheduled hours Monday-Friday, 9:00 a.m. - 6:00 p.m.
Passion for serving older adults and individuals living with dementia.

Core Responsibilities

Provide a safe, welcoming, and supportive environment for participants.
Deliver person-centered care that respects each participant's preferences, abilities, culture, and life experiences.
Assist participants with toileting, incontinence care, mobility, transfers, hydration, and other activities of daily living as needed.
Build meaningful relationships with participants through conversation, encouragement, and engagement.
Assist in the implementation of therapeutic, recreational, cognitive, social, and community-based activities.
Support participants during community outings and engagement opportunities.
Observe and report changes in participant condition, mood, behavior, or functioning to the Program Director.
Assist participants with meals, snacks, and hydration throughout the day.
Complete required documentation, care logs, and incident reports accurately and timely.
Participate in daily team meetings and monthly staff trainings.
Communicate professionally and compassionately with caregivers and family members.
Help create an inclusive and welcoming environment for participants and families from diverse cultural and linguistic backgrounds.
Support DayBreak's efforts to connect with and serve Spanish-speaking families within our community.
Assist with transportation of participants when authorized and qualified to drive DayBreak vehicles.
Maintain cleanliness, organization, and safety throughout the facility.
Adhere to all DayBreak policies, procedures, licensing requirements, and safety standards.
Perform other duties as assigned.

Additional Responsibilities

Assist Spanish-speaking participants and families with communication, program orientation, education, and ongoing support.
Support translation of participant needs, caregiver concerns, and program information when appropriate.
