Full Job Description

The Care Coordinator is responsible for coordinating and maintaining caregiver schedules for clients across all Pinnacle In-Home Care office locations based on operational needs. This position works closely with clients, independent contractors, families and internal staff to ensure continuity of care, timely staffing of visits, and exceptional customer service. As scheduling demands change, the Care Coordinator provides support where needed to ensure uninterrupted client services.

Work Schedule: Friday through Tuesday, 8:00 AM – 5:00 PM.

Physical Requirements

Prolonged periods of sitting while working at a computer.
Must be able to lift up to 15 pounds occasionally.

Essential Responsibilities

Coordinate and maintain caregiver schedules across all Pinnacle In-Home Care office locations as operational needs require.
Schedule, modify, and coordinate client visits to ensure continuity of care.
Verify client authorizations prior to scheduling services.
Proactively fill open and unassigned visits while minimizing service disruptions.
Communicate regularly with independent contractor caregivers, clients, family members, and agency staff.
Notify caregivers of schedule changes, new assignments, cancellations, and updates.
Accurately document all scheduling activity, client communications, caregiver communications, and scheduling changes within the scheduling system.
Process, document, and appropriately route client complaints in accordance with agency policy.
Monitor hospitalizations, discharges, service interruptions, and other changes affecting client schedules.
Collaborate with Human Resources, Nursing, Operations, and other Care Coordinators to ensure quality service delivery.
Review and respond to emails throughout the workday.
Maintain organized scheduling records and assist with administrative tasks as assigned.
Participate in weekly staff meetings and other required agency meetings.
Perform additional duties as assigned to support agency operations and client care.

Qualifications & Skills

Excellent organizational and time management skills with the ability to prioritize multiple tasks.
Strong verbal and written communication skills.
Ability to effectively coordinate schedules in a fast-paced environment.
Strong problem-solving and critical-thinking abilities.
Proficiency in Microsoft Outlook, Word, and Excel.
Ability to learn and efficiently utilize AxisCare and other agency software systems.
High level of professionalism, confidentiality, and attention to detail.
Ability to work independently while collaborating effectively with multiple office locations and departments.
