Full Job Description

Community Care Network is looking for an Intensive Direct Support Professional to join our team in Rutland, Vermont.

The Intensive Direct Support Professional (IDSP) provide hands-on, high-impact support to individuals with dual diagnoses who need intensive services. In the IDSP role, you’ll guide daily living activities, support appointments and community engagement, and help build real-world independence—from home routines to grocery shopping. You’ll deliver specialized behavioral support, contribute to care plans, and collaborate with a dedicated team to promote emotional wellness and meaningful progress.

Intensive Direct Support Professional Responsibilities:

Mentor, motivate, and advocate for individuals to support recovery, stability, and healthy social/emotional growth
Coordinate and implement individualized service plans with the broader care team
Respond calmly and effectively to behavioral and mental health crises
Use Therapeutic Options and de-escalation strategies to maintain safety
Follow specialized care procedures under RN guidance and seek consultation when needed
Create positive, engaging home and community experiences tailored to individual interests
Support medical visits and ensure continuity of care
Complete clear, timely documentation (notes, data tracking, timesheets)
Provide transportation as needed
Stay flexible to meet changing schedules, including weekends, holidays, and urgent coverage

Intensive Direct Support Professional Qualifications and Competencies:

High School Diploma or equivalent (GED) required.
Previous experience in developmental disabilities preferred.
Experience with Trauma Informed Care preferred.
Ability to work with others in a positive, responsible manner.
Strong writing and organizational skills in order to receive and convey information in efficient and effective manner.
Ability to maintain confidentiality.
Flexibility, dependability, and ability to work independently.
Commitment to community inclusion and individual choice.
Able to flex schedule as needed.
Must have a valid driver's license, reliable transportation with personal automobile liability insurance coverage at or above the levels mandated by the State of Vermont.
Successful completion of a criminal history background check after hire.
