Full Job Description

Job Summary: Client Care Coordinators help clients in their journey toward self-sufficiency and permanent housing through role modeling and trauma-informed support using the values, principles, and tools of the Housing First model and other industry best practices by:

1. Coordinating the basic needs, health, and welfare of program participants;

2. Assisting participants in developing healthy basic living skills;

3. Providing engagement, basic assessment screenings, and resource and referral information related to education, employment, housing, mainstream benefits, physical/behavioral healthcare, and other needs as identified; and

4. Assisting with community integration and transportation needs.

: There are three (3) levels in the Client Care Coordinator position. The CCC I classification is the most entry-level and receives direction from the Program Manager. Employees designated as Level II and III may also provide additional guidance and instruction to the Level I classification, at the direction of the Program Manager.

JOB DUTIES:

Level I

Assist residents with program orientation and daily needs such as medications and transportation.
Assist residents to develop basic living skills and those necessary to successfully integrate into the community.
Display empathetic, respectful, and motivational behavior while encouraging residents to take responsibility for their lives.
Demonstrate high professional/ethical standards and work well with others as part of the treatment team.
Act as a positive role model, providing residents with guidance and support.
Assist participants with intake and orientation to the program and community.
Accurately monitor and document the self-administration of medications; ensure proper storage of all medications and maintain accuracy of participant medication chart.
Assist residents with arranging their own appointments and transportation.
Assist with resource and referral needs related to case management activities, including education, employment, legal, housing, health and wellness, etc.
Assist residents with referrals and appointments with community resources, coordinating these efforts with other program staff.
Assist/arrange and/or provide resident transportation for appointments.
Assist with daily room checks and/or searches.
Collaborate with the case management team to meet residents’ personal care needs.
Maintain accurate and up-to-date records.
Ensure that the group living atmosphere remains respectful and positive by interacting with residents in an empathetic, respectful, and motivational manner and serving as a role model when providing them with support and assistance in problem-solving and encouraging them to take responsibility for their lives.
Monitor the safety and security of participants and the facility.
Maintain resident confidentiality and high ethical standards.
Prepare for and participate in staff meetings.
Complete required training.
Under the supervision of qualified clinical staff, assist with monitoring and documentation of health status, which may include making medication available to client and taking vitals.
Monitor and ensure ongoing health and safety standards, including facility cleaning and sanitation.
Deliver honest and professional communication.
Consistently contribute to a safe and positive work environment.
Maintain regular attendance and timeliness.
Perform additional job duties that may be required to meet the needs of the program and or the department ensuring compliance & adequate services.

Level II

Perform Level I duties

· Act as Lead Client Care Coordinator providing additional knowledge, expertise, and guidance to Level I Client Care Coordinators.

· Support the Program Manager with daily operational requirements and compliance activities.

Level III

· Perform Level I & II duties

· Prepare for and participate in individual and group supervision, case review, and staff meetings.

· Coordinate health and medical care for residents as well as assist them in developing healthy basic living skills; pursuing educational and employment interests; and integrating into the community.

· Help clients in their recovery from substance use and co-occurring disorders through role modeling and trauma-informed support using the values, principles, and tools of the treatment program.

· Act as Lead Client Care Coordinator providing additional knowledge, expertise, and guidance to Level I & II Client Care Coordinators.

· Perform other duties as assigned by the leadership team.

Knowledge, Skills, and Qualifications:

Level I

· High school diploma or GED.

Knowledge of the helping professions.
Ability to relate maturely and professionally with diverse and marginalized populations.
Ability to protect client confidentiality in all aspects of work.
Basic computer skills and experience, preferably w/Microsoft Word, Outlook, Explorer, and Google.
A valid driver’s license.
Preferred qualifications include knowledge and/or experience with addictions and co-occurring disorders.

Level II

High school diploma or GED and one (1) year of related work experience.

Level III

High school diploma or GED; Bachelor’s Degree and/or CRMA/CCAPP certification preferred and one (2) years of related work experience.

Special Requirements:

· Must possess a current and valid California driver’s license, including a driving record acceptable to F&H insurance company.

· Must pass criminal background screening.

· Must meet minimum requirements to access and use program data entry systems.

· Completion of a pre‐employment drug screening and post‐employment drug or alcohol tests upon

reasonable suspicion of use.

· Must be insurable by F&H insurance carriers.

· Available for call‐back to program as needed.

· Proof of legal right to work in the United States.

· T.B. clearance from a medical professional.

· Must be able to support a flexible schedule, including evenings, nights, and weekends.

· Current CPR/First Aid Certification

Typical Working Environment:

This is an on-site position performing work in a central location. The anticipated noise level is quiet with occasional dialogue or incoming calls, with no notable noise contributors. This position will work in close proximity to other people.

Equipment/Tools used:

Interaction/Supervision:

This position interacts with Directors, board members, managers, coworkers, the general public, vendors, clients, and families. Level II & III perform some oversight over Level I employees under the direction of the Program Manager.

Physical Demands:

Constantly (more than 66% per week): Talking, hearing, sitting, walking on normal surfaces, using a keyboard, fine dexterity with hands, grasping, and holding.

Occasionally (1% - 33% per week): Reaching outward, standing.

Rarely (less than 1 hour per week): Lifting/carrying up to 20 pounds, pushing and pulling up to 20 pounds, reaching overhead, kneeling, bending, and squatting.

Hours of Work:

This position will work the hours necessary to meet the needs of the business and to complete necessary

job duties. This may include work during days, evenings, nights, weekends, and holidays.
