Full Job Description

***NOW HIRING IMMEDIATELY *** For a part-time with the opportunity for full-time social services and mental health worker in the Sacramento area. Masters degree required.

PASSPORT TO LEARNING, INC. is seeking individuals with backgrounds in the social services and mental health fields who have the hearts necessary to work with people who have intellectual disabilities by using case management knowledge and other skills.

PASSPORT TO LEARNING, INC. (PTL) is dedicated to serving a community of individuals with
mental health intellectual disabilities who need assistance in adapting to meet age-specific developmental milestones. PTL's program is an age-appropriate functional skill training for individuals with Autism, MMR, Down Syndrome, and other intellectual disabilities. The focus of the practice is to utilize social services skills such as case management and other skills and training to increase a person's daily living abilities in their homes or place of residence and in the community, to pursue academic goals, or to explore employment options.

Candidates must be able to drive to meet with clients at their homes and provide mental health and social services support with attending court-mandated appointments and training in the community weekly. 95% of the work is performed in the field. Case management is included. Only candidates residing within 25 miles of one of the specified service areas are considered for this position. The work is performed in Sacramento and surrounding cities potentially depending on the need.The job also requires attending meetings two times per month at Sacramento office for submitting progress documentation, attending staff meetings, training, supervision, and client QA meetings. On-the-job training is provided.

*Compensation: F-AST payrate: ($42.00 MA/MS) per hour, plus mileage reimbursement at $0.76 cent/mile and travel time at California's minimum wage rate.

* Part-time position with possibility for additional hours when additional hours are available.

SKILLS:

* Case Management

*Progress Report Writing

* Knowledge of intellectual disabilities

* Ability to do report writing to state agencies

*Effective written and verbal communication skills

MINIMUM QUALIFICATIONS: staff education, psychology, counseling, nursing, social work, ABA, behavioral medicine, speech and language, rehabilitation, marriage and family therapy, or a comparable degree in a human services field such as criminal justice. Must have one year work experience rending services to individuals that have special needs.

ADDITIONAL REQUIREMENTS: Reliable transportation is a must. Valid California Driver’s License, Automotive Insurance, and CPR/ 1st Aid are required and background clearance.

(PTL) has delivered uncompromisingly effective life skills education, training, and support services in the social services and mental health industry to individuals with intellectual disabilities and other closely related intellectual /developmental disabilities (ID/DD). PTL’s services are offered through referrals from various Regional Centers throughout Southern and Northern California.

PASSPORT TO LEARNING, INC. (PTL) is dedicated to serving a community of individuals with intellectual disabilities that need assistance in adapting to meet age-specific developmental milestones. Independent Living Skills (ILS) and Adaptive Skills Training (AST) are age-appropriate functional skills training for individuals with Autism, Down Syndrome, and other intellectual disabilities to increase their daily living skills at home and in the community. Parenting Support Services (PSS) work one-to-one with ‘special needs’ parents in a mentor-style / instructor relationship to teach parenting skills. The judicially Involved Support Services program (JISS) provides support and guidance to clients in a mentor-style relationship to foster self-esteem and to assist clients in becoming productive and confident members of society. Supported Living Services (SLS) give support and functional skills training in various areas of daily living, such as Menu Planning, Meal Preparation, Money Management, Home Maintenance, Community Safety, and Resources, Access to Health Care.

Responsibilities

Complete assessment interviews and reports and co-facilitate Anger Management group in Sacramento.
Provide counseling and support to individuals with autism and developmental disabilities, utilizing therapy techniques.
Conduct assessments and develop treatment plans that address gender issues and mental health needs.
Collaborate with multidisciplinary teams to ensure comprehensive care and effective communication.
Maintain accurate documentation and records of client progress and treatment interventions.
Stay informed about advancements in psychopharmacology and integrate relevant knowledge into practice.
Engage in ongoing professional development to enhance skills and knowledge in social services and mental health.
