Site Manager (Enhanced Care Management) Direct Hire – Enhanced Care Management Program

CSL Staffing is seeking an experienced Site Manager- Enhanced Care Management to oversee a new Sacramento location and support the successful operation of a statewide Enhanced Care Management (ECM) Program.

This is a senior healthcare operations and program leadership role responsible for ensuring the site, care teams, patient services, compliance requirements, and business operations work together effectively.

The Site Manager- Enhanced Care Management will lead multidisciplinary teams and ensure individuals with chronic illnesses, behavioral health needs, and complex social challenges receive coordinated physical, behavioral, and social services.

Location: Sacramento, CA
Salary: $80,000–$85,000, depending on experience
Schedule: Monday–Friday, 8:30 AM–5:00 PM

Site Manager- Enhanced Care Management Key Responsibilities

Site and Team Leadership

· Oversee the daily operations of the Sacramento location.

· Lead, support, and manage multidisciplinary healthcare and care coordination teams.

· Conduct regular staff meetings, training sessions, and program updates.

· Provide guidance and oversight to ensure employees are meeting performance and quality expectations.

· ECM Program Oversight

· Oversee the delivery and ongoing operation of the Enhanced Care Management Program.

· Ensure patients receive a seamless continuum of coordinated healthcare, behavioral health, and social services.

· Work with care coordination and healthcare teams to ensure a Health Action Plan is created for each ECM patient.

· Monitor progress to ensure the goals outlined in each patient’s Health Action Plan are being addressed.

· Develop and implement care coordination strategies that improve patient engagement and outcomes.

· Oversee the development, implementation, and training of ECM policies, procedures, and protocols.

Compliance and Documentation

· Stay current on ECM rules, regulations, and health plan requirements.

· Ensure ongoing program compliance across all areas of responsibility.

· Oversee accurate and timely documentation within the electronic health record system.

· Ensure documentation supports high-quality patient care, program requirements, and timely reimbursement.

· Identify documentation or compliance concerns and work with staff to resolve them.

Partnerships and Communication

· Serve as a key liaison between internal departments, healthcare providers, health plans, referral partners, and community organizations.

· Develop and maintain effective systems of care coordination.

· Build strong professional relationships with health plan partners and external service providers.

· Coordinate with internal and external teams to ensure high-risk patients receive the services and resources they need.

Data and Performance Management

· Review productivity, quality, documentation, and patient outcome data.

· Extract and analyze data from the ECM system, EHR, and other reporting platforms.

· Use data to identify trends, operational challenges, and opportunities for improvement.

· Provide meaningful program and performance information to leadership and appropriate staff.

· Support initiatives that improve quality, efficiency, compliance, and patient outcomes.

· Perform other duties as assigned.

Qualifications

· Bachelor’s degree in healthcare, social services, business administration, or a related field, or equivalent relevant experience.

· Seven or more years of experience in a healthcare setting.

· Proven experience leading diverse teams and managing complex healthcare or care coordination programs.

· Strong knowledge of healthcare operations, program compliance, documentation, and performance management.

· Experience working with high-risk or vulnerable populations, including individuals experiencing homelessness or those who are justice-involved.

· Strong leadership, organizational, communication, and problem-solving skills.

· Ability to work effectively with health plans, healthcare providers, community organizations, and internal leadership.

· Experience working with EHR systems and healthcare data reporting.

Qualifications

Previous Enhanced Care Management or care coordination experience.

Experience working with Medi-Cal populations and health plan partners.

Bilingual English and Spanish required
