Full Job Description

Position Summary: The Home Visitor shall serve as the family’s primary contact with the Strong Families Strong Generation (SFSG) program. Under the supervision of the Team Leader, the Home Visitor is responsible for developing a relationship with the family and working in their home and community to build upon strengths and to enhance self-sufficiency. The Home Visitor will be tasked with providing culturally responsive services and referral/resources to enhance the social and emotional development of Black and African American families with children 0-5 years. The Home Visitor acts as a communication and trust bridge between the family, child protective services, social services and mental health systems. All duties will be performed within a strength-based, culturally responsive, and trauma informed lens and as part of MAN’s focused work in Black Child Legacy Campaign (BCLC) and Reducing African American Child Death (RAACD).

This position is non-exempt, hourly, and full time.

Compensation: $23-$25/Hour

Essential Functions:

1. Home Visitation

Support the efforts of the Black Child Legacy Campaign with a focus on the reduction of infant sleep-related deaths and the prevention of child abuse and neglect.
Conduct regularly scheduled home visits to assigned families, to provide the Effective Black Parenting Program to parents and children in their homes.
Conduct screens and assessments with all assigned families.
Work with families to empower them to engage in problem solving.
Model parenting behavior, and provide caregivers with support, education/information, and age-appropriate parenting and positive discipline techniques.
Promote caregiver participation in home activities designed to promote the physical, emotional, social, and intellectual development of their children.
Work with families to develop, review, and update goal plans per policies and procedures under the direction of the Team Leader.
Model nurturing attitudes and behaviors in all contacts with program participants and program staff.
Utilize community resources, in-home teaching, and supportive problem solving to help families meet their goals.
Advocate for and act as a liaison for families with existing community services and agencies, which may include routine translation and interpretation.

2. Supervision, Multi-Disciplinary Resource Team Case Reviews, Training

Attend weekly supervision with the Team Leader.
Participate as a member of the SFSG Multi-Disciplinary Resource Team (MDT) of service providers, presenting assigned families to the MDT as required.
Consult with MDT members regarding issues the family may experience that need additional assessments, referrals, or services.
Facilitate and document ongoing progress of the MDT action plan.
Attend required trainings provided by The Child Abuse Prevention Center, the County of Sacramento, and its Contractors.

3. Case File Documentation

Complete documentation as required by B&B Policies & Procedures; including but not limited to: Face to Face reports; goal plans; Multi-Disciplinary Resource Team forms; Quarterly Health Records; Critical Incident Reports, CPS reports, etc.

4. Other Functions:

Assist with and participate in outreach, community engagements, referrals, and volunteer generation.
Maintain a safe, clean, resourceful, and enriching environment for families and staff.
Assist with, conduct and/or participate in Mutual Assistance Network’s Community Centers (also referred to as Family Resource Center) classes, events, and activities. Assist as needed with play care.
Participate in health insurance outreach and Medi-Cal Administrative Activities (MAA) claimable activities as defined in the MAA claim plan, and accurately complete MAA time studies.
Perform other duties as assigned that are associated with the B&B Performance Measures.
Must report to work site as scheduled and agreed upon with Supervisor (Team Leader).
Other duties as assigned.

Knowledge, Skills and Abilities:

Must be able to read, write, speak and understand the English language.
Ability to work with diverse workgroups and serve families with diverse economic, social, racial, and cultural backgrounds.
Knowledgeable about local community resources.
Ability to establish and maintain personal and programmatic boundaries while providing supportive services.
Strong interpersonal skills and the ability to relate to individuals who may not share basic commonality, including value systems and behavior norms.
Experience in working with culturally diverse communities and families, with the ability to be culturally sensitive and appropriate.
Demonstrate good writing and organizational skill.
Skill to maintain a professional, confidential work environment.
Ability to manage multiple tasks in an efficient manner.
Knowledge of child abuse issues, substance abuse and family dynamics.
Knowledge of child development and behavior.
Ability to work evenings and weekends, as required.
Ability to communicate clearly both orally and in writing.
Experience in social services in community settings with families preferred.
Able to deal with stressful situations.

Compensation : Non Exempt $23-$25/hour

Physical Qualifications

Ability to work in office setting and operate basic office equipment (phones, fax and copy machine)
Ability to perform basic computer functions (word processing, internet research, email
Ability to work with individuals in their home environment
Must be able to lift and carry up to 15 lbs.
Ability to frequently sit, bend, twist, squat, kneel, reach, push and pull
Ability to occasionally walk up to one mile intermittingly with breaks
Ability to climb stairs

Other Qualifications:

Must be at least 18 years of age.
Must be a U.S. citizen or lawful permanent resident.
Clearance of fingerprint background check.
Must possess a high school diploma or GED.
Must possess a valid California Driver’s license, reliable transportation and auto insurance.
Able to travel between sites and to offsite events.

Job Type: Full-time

Pay: $23.00 - $25.00 per hour
