Full Job Description

Overview
DoubleTree Post Acute is seeking a highly motivated and detail-oriented MDS Coordinator to join our healthcare team. In this vital role, you will lead the comprehensive assessment and documentation process for residents in long-term care facilities, ensuring compliance with federal and state regulations. Your expertise will support optimal patient care, accurate medical records, and seamless coordination across multidisciplinary teams. This position offers an exciting opportunity to influence quality outcomes and enhance resident well-being through meticulous clinical documentation and case management.

Duties

Conduct thorough Minimum Data Set (MDS) assessments for residents, utilizing EMR
Review and analyze medical records, clinical documentation, and healthcare data to determine appropriate coding using ICD-10, ICD-9, CPT, DRG, and other coding standards
Collaborate with interdisciplinary teams—including nursing, therapy, social work, and physicians—to develop individualized care plans aligned with NCQA standards and Medicare requirements
Ensure compliance with HIPAA regulations by maintaining confidentiality of patient information while managing medical records and documentation review processes

Qualifications
Proven experience as an MDS Coordinator or in a similar role within long-term care facilities or hospitals with acute or critical care units
Strong knowledge of medical terminology, anatomy, physiology, and healthcare coding standards including ICD-10, ICD-9, CPT coding, DRGs, and Medicare regulations
Familiarity with managed care processes, utilization review/management, discharge planning, and health information management systems
Experience working in a Skilled Nursing Setting
Proficiency or familiarity with (EMR) Point Click Care
Excellent organizational skills with attention to detail for documentation review and medical records management
Knowledge of HIPAA compliance standards and best practices for safeguarding patient information
Background in nursing or clinical case management with critical care experience preferred; pediatrics or primary care experience is advantageous
Ability to communicate effectively across multidisciplinary teams while maintaining a positive attitude focused on quality improvement
