Case Manager/Social Worker – $5000 Sign on (Hybrid)
Location: Hybrid / Remote – Mt. Pleasant/Clare County
Schedule: Full-time | Flexible schedule - based on satisfactory performance
Bonus: $5,000 sign-on bonus
Salary: $55,000–$70,000 depending on license and experience

Position Summary
The Social Worker / Supports Coordinator provides MI Choice Waiver case management services, including assessments, service coordination, monitoring, and documentation. This role requires independent time management, professional judgment, and consistent performance to maintain hybrid work flexibility.
Basic Job Duties
- Complete accurate and timely documentation in compliance with program, regulatory, and organizational requirements
- Communicate effectively with office staff, support coordinators, healthcare providers, and community agencies
- Conduct required home visits and community-based assessments within the assigned service area
Requirements
- Licensed Social Worker in the State of Michigan (LLBSW, LBSW, LLMSW, or LMSW)
- Understanding of confidentiality and HIPAA regulations
- Strong interpersonal, organizational, and communication skills
- Ability to work independently while meeting productivity and quality standards
Preferred Qualifications
- One year or more of social work or case management experience
- Experience in home health care, waiver services, or community-based programs
- Knowledge of healthcare regulations and quality management principles
