LPN/ RN – Home Health Visits | Tucson Area

Position Summary

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to provide skilled nursing visits to patients in the Tucson area. This position involves providing care in the patient’s home while following the plan of care, physician orders, agency policies, and documentation requirements.

This is a great opportunity for an LPN who is interested in starting part-time or PRN, with the potential to transition into a fuller schedule as patient assignments become available in the Tucson area.

Location

This position is specifically for the Tucson, Arizona area.

Candidates must be located in or near Tucson and have reliable transportation to complete home health visits in the assigned area. Orientation and onboarding may include a combination of remote training, document completion, and coordinated agency instruction prior to independent patient assignment.

Key Responsibilities

Provide skilled nursing care in the patient’s home according to the plan of care.
Perform nursing services such as insulin administration, catheter care, wound care, diabetes management, medication management, and other ordered interventions.
Complete patient assessments and report changes in condition as appropriate.
Educate patients and families regarding medications, treatment plans, disease management, and safety precautions.
Follow all agency policies, patient-specific instructions, and physician orders.
Complete accurate and timely documentation for each visit.
Comply with Electronic Visit Verification requirements.
Communicate professionally with patients, families, office staff, clinical leadership, and members of the care team.
Report incidents, patient refusals, changes in condition, missed visits, delays, or concerns according to agency policy.
Maintain professionalism, confidentiality, and patient dignity at all times.
Respond appropriately to urgent patient needs or emergencies.

Schedule

Part-time and PRN opportunities available.
Flexible/open availability is preferred to help support patient scheduling needs.
Sunday through Saturday availability preferred.
Weekend availability is needed depending on patient assignments.
Schedule may vary based on patient needs, nurse availability, and assigned Tucson-area patient schedule.
Potential to transition into a fuller schedule as additional Tucson-area patient assignments become available.

Qualifications

Current and unrestricted Licensed Practical Nurse license in the State of Arizona.
Current CPR/BLS certification.
Valid Arizona Fingerprint Clearance Card.
Article 9 certification, or willingness to obtain prior to patient assignment.
At least one year of home health, private duty, developmental disability, or related nursing experience preferred.
Knowledge of Electronic Visit Verification preferred.
Strong communication and documentation skills.
Ability to work independently in a home health setting.
Reliable transportation.
Valid driver’s license
Ability to manage time effectively and follow scheduled visit times.
Comfortable using technology for communication, documentation, and electronic forms.
Ability to use Microsoft Teams, Kinnser/WellSky EMR, Adobe electronic forms/signatures, and other agency-required systems.
Excellent organizational skills and attention to detail.
Commitment to providing high-quality care to individuals with developmental disabilities and/or complex care needs.

Physical Requirements

This role may require the ability to assist with patient positioning, transfers, mobility support, and other physical tasks related to patient care. Nurses must be able to safely perform assigned duties in the home setting and request assistance when needed.
