Director of Nursing (DON) – Home Health
Full-Time | St. Augustine, FL
Lead a Growing Home Health Team

Nayar Health Care St. Augustine is seeking an experienced Director of Nursing (DON) to lead our Home Health clinical team. Located in St. Augustine, Florida, our office proudly serves patients throughout Northeast Florida by delivering compassionate, high-quality care in the comfort of their homes.As the Director of Nursing, you will oversee daily clinical operations, support and develop your clinical team, ensure regulatory compliance, and drive quality patient outcomes.

This is a hands-on leadership role where you will provide direct patient care as needed while helping grow the agency by mentoring clinicians, supporting referrals, and building a high-performing clinical team.

Responsibilities

Provide leadership and oversight of all clinical operations.
Deliver direct patient care and complete home visits as needed while building census.
Recruit, mentor, and develop a high-performing clinical team.
Ensure compliance with Medicare Conditions of Participation, state regulations, and agency policies.
Oversee patient admissions, case assignments, and care coordination.
Lead quality assurance and performance improvement initiatives.
Conduct staff orientation, competency evaluations, and performance reviews.
Collaborate with Human Resources to recruit and retain top clinical talent.
Promote exceptional patient outcomes and a culture of accountability.

Qualifications

Active Florida Registered Nurse (RN) license.
Minimum one year of Home Health leadership or supervisory experience.
Strong knowledge of Medicare regulations, OASIS, and Home Health operations.
Bachelor's degree in Nursing (BSN) preferred.
Current CPR certification.
Excellent leadership, communication, and organizational skills.
