PRN Pharmacist – Infusion/Specialty | Vital Care

Vital Care is seeking a licensed PRN (as-needed) pharmacist to support our infusion and specialty pharmacy operations on a flexible, per-diem basis. In this role, you will review and verify physician orders for home and ambulatory infusion therapies—including IV antibiotics, parenteral nutrition (TPN), IVIG, biologics, hydration, and other specialty medications—while ensuring accuracy, appropriate dosing, and safe compounding practices. Day-to-day responsibilities include overseeing sterile compounding in compliance with USP <797> and <800> standards, conducting clinical monitoring and drug-utilization review, counseling patients and caregivers, coordinating with nurses and prescribers on therapy plans, and maintaining accurate documentation in accordance with state and federal regulations. As a PRN team member, you'll provide coverage during peak periods, staff absences, and weekends or holidays as needed, making reliability and adaptability essential.

The ideal candidate holds an active pharmacist license in good standing (PharmD or BS in Pharmacy) and has prior experience in home infusion, specialty, or hospital/IV pharmacy settings, with a strong working knowledge of sterile compounding and aseptic technique. Familiarity with infusion therapy management, payer/reimbursement basics, and pharmacy software systems is a plus. We're looking for a detail-oriented, patient-focused professional who communicates well across a multidisciplinary care team and can work independently with minimal supervision.
