Full Job Description

Description:

TriCity Family Services, a dedicated community mental health organization in Geneva, is seeking a compassionate and organized Psychiatric Case Manager to join our team. This role offers an opportunity to make a meaningful difference in the lives of individuals and families by providing essential support and coordination within our mental health services.

As our Psychiatric Case Manager, you will bridge the gap between clients, families, and our psychiatric team by streamlining care, administrative workflows, and clinical support. In this vital role, you will conduct intake assessments, manage client vitals, handle care coordination, and provide frontline administrative support to ensure high-quality, culturally sensitive service delivery.

Requirements:

Conducts client intake assessments and provides critical referral and linkage services to individuals and families.
Coordinates directly with psychiatric prescribers and pharmacies to facilitate efficient medication management and refill requests.
Provides dedicated assistance and administrative coordination support to the clinical and psychiatric teams.
Proactively follows up with clients, parents, or guardians regarding appointments, medication compliance, and care management. Completes specialized Medication Administration Consent Forms for schools upon parent/guardian request.
Maintains accurate, compliant, and timely electronic medical record (EMR) documentation.
Serves as reliable front-desk backup support, including answering multi-line phones, greeting and checking in clients, and collecting fee payments.
Upholds absolute client confidentiality (HIPAA compliance) and treats all individuals with deep cultural sensitivity.
Performs other duties and operational tasks as assigned.

Qualifications

Employment Type: Part-Time (up to 29 hours per week), with the potential to transition to Full-Time.
Experience: Prior experience with medical scheduling, case management, or medical office coordination is preferred.
Education: High school diploma is required.
Language Skills: Bilingual proficiency in both Spanish and English is preferred; exceptional interpersonal, oral, and written communication skills.
Technical Skills: Highly comfortable using computers, EMR tracking systems, telehealth platforms, and Microsoft Office Suite (Word, Excel, Outlook).
Core Competencies: Strong organizational skills with an ability to manage multiple tasks; exceptional attention to detail; self-motivated to work both independently and collaboratively.
Professionalism: Courteous, customer service-oriented mindset with the ability to handle highly sensitive, confidential clinical data with utmost discretion.
