Bereavement Coordinator, Inpatient Hospice
Part time
8 a.m. to 4:30 p.m.
Develops and implements the bereavement program. Oversees bereavement plans for each patient and family. Provides group support and individual counseling to bereaved clients per bereavement plan of care.
- Assesses bereavement needs and provides bereavement follow-up to each bereaved referred to the Bereavement Program.
- Plans, develops, and implements Bereavement Support Groups and educational workshops on specific grief issues. Plans Memorial Service(s). Participates in Team meetings.
- Participates in in-service, staff and other agency meetings for the purpose of planning, coordinating and generally improving the quality of service provided by the agency.
- Participates in the orientation and training of new employees to the Bereavement Program. Develops educational materials and resources for distribution to families and staff.
- Ensures that required documentation for activities within areas of responsibility is completed in an accurate and timely
- Actively participates in safety initiatives and risk mitigating measures where appropriate and completes all position and unit safety related competencies and requirements on a timely basis.
- Performs other duties as assigned.
Required
- Master’s degree in social work, Counseling Psychology, or Nursing
- Experience with grief and loss.
- Driver’s license, State of Michigan
