Health Dimensions Group is seeking an experienced Regional Director of Operations, Transitions to join our Operations Leadership Team. In this highly visible leadership role, you’ll lead the operational execution of new developments and managed-community transitions, serving as a key point person from initial planning and onboarding through successful integration and handoff to ongoing operations.
This role combines project leadership, senior living operations, financial oversight, leadership development, and transition management across skilled nursing, assisted living, and independent living environments.

Serve as the operational project manager and primary point person for new developments and managed-community transitions.
Lead the planning and execution of community onboarding and offboarding, ensuring transition plans and action items are successfully carried through.
Conduct regular site visits throughout transitions to support implementation of core operational processes and adherence to applicable regulatory standards.
Assess and onboard community leadership teams prior to openings or management transitions while establishing alignment with HDG’s values, expectations, and operational standards.
Facilitate the successful handoff of newly developed and transitioned communities to the appropriate Regional Director of Operations and ongoing HDG operations team.
Develop, implement, and monitor transition and new-development pro formas and budgets in partnership with ownership and Finance.
Provide operational oversight to ensure communities are positioned to achieve financial expectations, positive operating margins, and long-term sustainability.
Analyze community operations and performance to identify opportunities for increased efficiency, cost reduction, and operational improvement.
Develop and implement corrective action plans to address operational or departmental gaps.
Establish systems, processes, and controls that create a strong operational foundation throughout the transition.
Coach, mentor, and support Executive Directors and community leadership teams throughout periods of transition.
Assess leadership needs and help establish strong, accountable teams prepared to successfully lead the community beyond the transition period.
Partner with People & Culture on recruitment, retention, leadership onboarding, and orientation strategies for key positions.
Identify and address staffing structures and workforce challenges that may impact transition success.
Foster a culture of accountability, teamwork, trust, communication, and collaboration across transitioning communities and support teams.
Ensure transitioning communities operate in alignment with applicable policies, regulations, laws, and organizational standards.
Monitor regulatory and policy changes that may impact community operations and advise internal and external stakeholders accordingly.
Develop and present operational and compliance reporting to HDG leadership, ownership groups, Boards, and other stakeholders.
Attend Board and investor meetings for new developments and transitioning communities as needed, providing updates on operational results, strategies, and key initiatives.
Maintain sound judgment, confidentiality, and ethical decision-making throughout all transition activities.
Partner across HDG to evaluate and support new growth, acquisition, and managed-community opportunities.
Maintain awareness of industry and senior services trends to help guide new service development and operational planning.
Build trusted relationships with ownership groups, clients, Boards, vendors, community leaders, and other key partners.
Represent Health Dimensions Group at industry meetings, conferences, and professional events.
Bring a strategic, solutions-oriented approach to complex operational challenges while ensuring decisions align with HDG’s long-term objectives.
This is a remote/home-based leadership position with significant travel to new developments and communities undergoing onboarding, offboarding, or operational transition.
The role requires up to 80% travel, including overnight stays, based on transition activity and organizational needs. We’re looking for a leader who is comfortable being highly present in communities and can adapt quickly as priorities, projects, and locations evolve.
Bachelor’s degree in Healthcare Administration, Business, or a related field required; Master’s degree preferred.
Minimum of 10 years of progressive leadership experience within senior living, skilled nursing, or senior housing.
Proven success leading high-volume multi-site operations with full P&L responsibility.
Demonstrated experience leading operational turnarounds and large-scale performance improvement initiatives.
Experience mentoring and developing leaders at the regional and/or corporate level.
Recent, hands-on experience leading new developments, acquisitions, community transitions, or M&A integration within senior living is required.
Active involvement or leadership within industry boards, associations, or professional organizations is preferred.
Comprehensive understanding of senior living and skilled nursing operations, healthcare regulations, and financial frameworks.
Strong financial acumen with experience managing budgets, analyzing operational performance, and using data to drive decisions.
Ability to quickly assess complex operational environments, identify priorities, and translate findings into actionable transition plans.
Strong project management skills with the ability to lead multiple priorities and transitions simultaneously.
Proficiency with Microsoft Office and operational reporting tools.
Strong executive presence with exceptional communication and relationship-building skills.
Proven ability to coach, mentor, and develop high-performing leaders and teams.
Strategic, systems-level thinker who can balance immediate operational needs with long-term organizational goals.
