Care Manager – Americare Behavioral Health Adult Foster Care PER VISIT

Position: Care Manager
Program: Americare Behavioral Health Adult Foster Care
Reports to: Program Manager
Location: Adult Foster Care member homes, community-based settings, and administrative office as needed
Employment Type: Full-time, part-time, or per diem options may be available

About the Role

Americare Behavioral Health Adult Foster Care is seeking a compassionate, organized, and member-focused Care Manager to support elders and adults with disabilities enrolled in our Adult Foster Care program. The Care Manager works as part of the multidisciplinary professional team with nursing staff, members, caregivers, families, and community providers to support person-centered care, member rights, safety, service coordination, documentation, and ongoing program compliance.

Key Responsibilities

· Participate as a member of the Adult Foster Care multidisciplinary professional team and collaborate with nursing staff to support each member’s AFC plan of care.

· Complete psychosocial, functional, member rights, environmental, and service-related assessments in accordance with program requirements and agency policy.

· Support the development, implementation, review, and revision of individualized care plans in collaboration with the member, caregiver, family, nurse, and care team.

· Conduct required home visits and ongoing monitoring to assess member well-being, caregiver support needs, safety, service delivery, and changes in condition or circumstances.

· Assist with care coordination, referrals, community resources, benefits navigation, and communication with external providers as appropriate.

· Educate members and caregivers regarding program expectations, member rights, reporting responsibilities, emergency planning, service coordination, and available supports.

· Identify and promptly report concerns related to abuse, neglect, exploitation, safety, caregiver availability, environmental risks, incidents, accidents, or changes that may affect AFC eligibility or care needs.

· Prepare timely and accurate documentation, progress notes, assessments, care plan updates, visit records, and required program reports.

· Participate in case conferences, interdisciplinary meetings, quality management activities, record reviews, trainings, and staff meetings.

· Maintain confidentiality, professional boundaries, cultural competence, and compliance with HIPAA, MassHealth AFC requirements, agency policies, and applicable state and federal regulations.

Massachusetts Adult Foster Care Care Manager Qualifications

· Track 1 (Licensed): Bachelor’s degree in a human services field and a valid Massachusetts Social Work License (LSW, LCSW, or LICSW), with at least two years of recent experience working with elders or adults with disabilities. OR

· Track 2 (Unlicensed / Clinical): Bachelor’s degree from an accredited four-year college or university and at least two years of direct clinical experience caring for elders or individuals with physical, intellectual, or developmental disabilities.

· Must meet all applicable Adult Foster Care staff requirements, including reference and job history review, CORI, SORI, OIG screening, applicable license or certification verification when relevant, and tuberculosis screening as required for AFC staff.

· Must not be related to or legally responsible for any member receiving Adult Foster Care services from Americare Behavioral Health Adult Foster Care.

· Ability to recognize and report suspected abuse, neglect, exploitation, incidents, accidents, and safety concerns involving elders and adults with disabilities.

· Strong assessment, documentation, communication, time management, problem-solving, and care coordination skills.

· Ability to work independently in member homes while collaborating effectively with nursing staff, caregivers, families, supervisors, and community providers.

· Valid driver’s license, reliable transportation, and ability to travel to member homes, meetings, and community locations.

Work Environment and Physical Requirements

This position involves supporting Americare Behavioral Health Adult Foster Care members in their residences and traveling to various work sites. The Care Manager may work in home and community environments that vary in accessibility and may require walking, standing, sitting, reaching, using phones and computers, bending, stooping, climbing stairs, carrying documents or equipment, and communicating with members, caregivers, families, and providers in person and by phone.
