JOB TITLE: Community Integration Specialist
UNA LOCATION: MASCO, Inc. - Boardman, OH
PAY RANGE: $19 - $23 per hour
WORK SCHEDULE: Mon-Fri day shift. No nights, no weekends, no holidays.

SUMMARY: The Community Integration Specialist develops, plans & schedules activities in the community for clients to participate in during day services at all UNA locations. This includes promoting events and scheduling staff to supervise activities. The Community Integration Specialist must be able to work as a team with other departments to secure vehicles, plan routes and ensure proper supervision levels are retained.
ESSENTIAL DUTIES:
- Develop and plan activities in the community that are enjoyable experiences for our clients
- Train staff on ISP details and retrain when plans are revised/renewed
- Attend planning meetings and maintain current files on clients for participation in events
- Ensure proper staffing ratios for all events to maintain client safety
- Work with Transportation Department to secure company vehicles and plan routes for events
- Maintains and submits incident reports, documentation, mileage and trip reports and other documentation as required by DODD rules, state law, and company policy
- Performs billing functions that include gathering service delivery documentation and submission for reimbursement.
- Reviews QA reports and responds to recommendations
- Reviews and reports Unusual and Major Unusual Incidents as required. Monitors Incident Reports for identifiable patterns and preventative measures to reduce or eliminate concerns
- May be called upon to gather data information for the development of behavior support strategies. Responsible for crisis intervention
- Attends in-service training, seminars, and other meetings as required
- May be required to take clients on trips if needed
- Other duties as assigned
QUALIFICATIONS and CHARACTERISTICS:
- High School Diploma or GED
- Experience as Outcome Specialist or DSP preferred
- 2 years experience working with adults with developmental disabilities
- Experience taking clients on community outings or HPC
- Valid drivers license with less than 6 points; ability to travel to other locations within region
- Basic knowledge of Microsoft Office products (Outlook, Word, Excel, PowerPoint)
- Must be able to multi-task and prioritize work

- Ability to adapt to changes and spur of the moment decision-making
- Responsible, accountable, dependable team player
PHYSICAL REQUIREMENTS: Must be able to stand, walk, lift, and move objects. Must be able to stoop, stretch, bend, and lift up to 50 pounds. Must be able to push clients in wheelchairs if necessary. May be required to physically restrain client if behavior is a danger to themselves or others. Must be able to sit to use computer and telephone. Eyesight capabilities must be sufficient to perform these functions. Hearing must be adequate to perform these functions.
OTHER REQUIREMENTS:
- Pass criminal background check
- Pass pre-employment drug screen
- Pass pre-employment physical exam
- Complete first aid and AED certification
- Complete all DODD training requirements
FLSA: Hourly non-exempt; eligible for OT pay
WORK SCHEDULE: Mon-Fri, 8 hours per day, and any additional time that is required to perform the job
PROBATIONARY PERIOD: 90 Days
