Full Job Description

Position Summary

The Home Health Clinical Manager is responsible for the overall clinical operations of the agency, ensuring the delivery of high-quality, patient-centered care while maintaining compliance with all federal, state, Medicare, and company regulations. This leadership role oversees clinical staff, coordinates patient care services, supports quality improvement initiatives, and partners with agency leadership to achieve operational, financial, and clinical goals. The Clinical Manager serves as a clinical resource for staff, promotes professional development, and fosters a collaborative, patient-focused culture.

Essential Responsibilities:

Clinical Leadership

Provide leadership and oversight for all clinical operations to ensure safe, effective, and compliant patient care.
Supervise Registered Nurses, therapists, home health aides, and other clinical personnel.
Direct and coordinate patient care services to ensure quality outcomes and continuity of care.
Review patient referrals and determine eligibility and appropriateness for home health services.
Oversee patient admissions, non-admissions, transfers, discharges, and coordination of care.
Assign appropriate clinicians based on patient needs, disciplines required, and geographic territory.
Assist clinicians in developing individualized Plans of Care (POCs), establishing treatment goals, setting priorities, and revising care plans as patient needs change.
Provide clinical guidance and support to staff during operating hours and serve as a resource for complex patient care situations.
Provide direct patient care on an infrequent basis during emergencies or staffing shortages.
Participate in the agency's on-call rotation.

Quality Assurance & Regulatory Compliance

Ensure all clinical documentation is accurate, complete, timely, and compliant with Medicare Conditions of Participation, payer requirements, and agency policies.
Review and approve clinical documentation, including Start of Care assessments, OASIS documentation, evaluations, recertifications, Plans of Care, and physician orders.
Conduct chart audits and monitor documentation quality, addressing deficiencies promptly.
Lead Quality Assurance and Performance Improvement (QAPI) activities, monitor quality metrics, and implement performance improvement initiatives.
Ensure the agency remains survey-ready and compliant with all state, federal, accreditation, and Medicare regulations.
Investigate and resolve patient, family, physician, and staff concerns in a timely and professional manner.

Staff Leadership & Development

Interview, hire, onboard, train, coach, mentor, and evaluate clinical staff.
Recommend employment decisions including promotions, transfers, disciplinary action, and termination to agency leadership.
Conduct performance evaluations and provide ongoing coaching to improve clinical performance.
Assess educational needs through documentation review, quality audits, and performance evaluations.
Develop and conduct staff education, in-services, case conferences, and competency training.
Foster employee engagement, accountability, and professional growth while promoting a positive team culture.

Operations Management

Oversee daily office and clinical operations to ensure efficient workflow and continuity of patient care.
Manage clinician scheduling, productivity, staffing levels, and workload distribution.
Oversee intake processes, referral management, payer verification, authorization, and precertification requirements.
Coordinate communication between physicians, referral sources, clinicians, patients, and caregivers.
Review payroll submissions, scheduling reports, productivity metrics, and operational reports.
Monitor billing readiness by ensuring documentation supports timely reimbursement.
Collaborate with agency leadership to establish operational goals, budget objectives, and financial performance targets.
Support marketing initiatives and maintain positive relationships with physicians, hospitals, referral partners, and community organizations.

Administrative Responsibilities

Participate in leadership meetings, case conferences, and interdisciplinary team meetings.
Maintain required reports, quality metrics, and operational benchmarks.
Assist with disaster preparedness planning and emergency response activities.
Serve as acting Branch Director or Administrator during their absence when designated.
Perform other duties as assigned to support agency operations.

Required Qualifications

Graduate of an accredited School of Nursing.
Current Registered Nurse (RN) license in the Oregon and CPR certification
Valid driver's license, reliable transportation, and proof of automobile insurance.
Minimum of two years of Registered Nurse experience.
Minimum of 2+ years of home health experience.
Thorough knowledge of federal, state, Medicare, and accreditation regulations governing home health agencies.
Strong leadership, organizational, communication, problem-solving, and critical thinking skills.
Proficiency with electronic medical records and Microsoft Office applications.

Preferred

Three or more years of home health experience.
Preceptor or clinical educator experience.
Previous experience leading Quality Assurance and Performance Improvement (QAPI) initiatives.
