Job Overview

FirstLight Home Care is an established and growing home care agency seeking an energetic and experienced RN Manager to help launch and lead our skilled home health services. This is a rare opportunity to join a stable organization with an existing reputation, referral network, and leadership team while playing a key role in building our skilled care division from the ground up.

You'll work alongside a supportive leadership team to develop clinical processes, establish best practices, mentor staff, and shape the future of skilled care within a well-established home care organization. Your expertise and leadership will directly influence the quality of care we provide and the success of our growing skilled nursing division.

Leadership & Management Responsibilities

Lead the development and growth of the skilled home health program.
Assist with recruiting, hiring, onboarding, and mentoring clinical staff.
Ensure compliance with state, federal, and agency regulations.
Participate in quality assurance and performance improvement initiatives.
Develop and implement clinical policies, procedures, and best practices.
Collaborate with agency leadership to support program growth and operational success.
Provide clinical oversight and support to nursing and caregiving staff.
Monitor quality outcomes and identify opportunities for improvement.

Clinical Responsibilities

Conduct nursing assessments and ongoing client evaluations.
Develop, implement, and revise individualized plans of care.
Provide medication management and medication administration as ordered.
Perform nursing delegation in accordance with state regulations.
Provide disease process education and training to clients and families.
Participate in remote patient monitoring programs.
Document assessments, supervisory visits, care plans, client education, and clinical interventions.
Communicate changes in client condition to appropriate parties.
Maintain accurate and timely clinical records.
Attend staff meetings, client care conferences, and in-service trainings.
Supervise and educate clinical personnel when applicable.
Perform other duties as assigned.

Qualifications

Current unrestricted RN license in Oregon.
Minimum three (3) years of recent Home Health or Hospice experience required.
Previous supervisory, management, or leadership experience strongly preferred.
Knowledge of OASIS documentation preferred.
Strong assessment, care planning, and clinical decision-making skills.
Excellent communication and organizational skills.
Current BLS (Basic Life Support) Certification required.
Valid driver's license, reliable transportation, and proof of insurance.
