Hospice RN Case Manager – Homecare

Location: Serving patients and families in Davie County and surrounding counties
Office Location: Trellis Supportive Care – Mocksville, NC

Schedule

Full-Time (40 hours per week)
Five 8-hour weekday shifts
Occasional back-up on-call rotation

Position Summary

The Hospice RN Case Manager – Homecare provides compassionate hospice care to patients in their homes and care facilities. This role is responsible for:

Conducting comprehensive nursing assessments
Developing and implementing individualized plans of care
Maintaining the highest level of patient comfort and quality of life
Coordinating care through an interdisciplinary team approach
Performing ongoing reassessments and revising care plans as patient/family needs evolve
Supervising and collaborating with team members to ensure exceptional care delivery
