Full Job Description

House of Hope (HOH) is searching for a Full-Time Case Manager to assist the women served by our outpatient program. Make a difference in people’s lives and our community! This role is critical to HOH clients and the organization. We will teach the successful candidate how to assess clients’ needs, connect them with appropriate resources, accurately document services, learn healthy boundaries, and maintain client safety. Coordinating care with clinical staff and becoming a direct line of support to our clients are key to success in this position. Help develop therapeutic interventions for clients, hold them accountable to their goals, and reward them for their excellent work.

Primary Responsibilities

Teach women’s groups (curriculum provided) on topics such as recovery education, relapse prevention, self-sufficiency, conflict resolution, relationships and communication, etc.
Link women with community resources that enrich their recovery process, including community health, medical, social services, and childcare providers.
Work professionally with community partners and coordinate with case workers from other agencies such as DCFS, Probation and Parole, etc.
Complete all necessary documentation with a high level of computer proficiency and time management.
Responsible for individual case management, meeting regularly with individual clients.
Document the results of assessment and treatment plans, write reports, progress notes, discharge summaries, and other client data; responsible for daily documentation on monthly tab sheets and month-end compilation of documentation and billing.
Ensure client files contain all relevant information including treatment needs and future needs.
Consult with other professionals regarding client treatment and services to assure comprehensive quality care for the client.
Monitor the client’s progress to ensure treatment compliance in acquiring necessary community-based services.
In conjunction with client, conduct needs assessments, continuous service planning, advocacy, and help with benefits acquisition and service linkages.
Effectively communicate both verbally and in writing; follow written and oral instructions.
Transport clients to appointments, as needed, to supervise or assist with case management issues.

Qualifications, Experience & Skillset

Adept at managing crises and resolving conflicts in a calm professional manner, able to balance multiple priorities and generally oversee the details of a healthy treatment community with the overall goal of empowering women and strengthening families.
Excellent communication skills for building rapport with clients and collaborating with a multidisciplinary team.
Commitment to ongoing professional development and staying informed about advancements in the field of mental health and substance abuse treatment.
Organized, innovative, and adept at problem-solving skills and able to adapt to the evolving needs of clients and the organization.
Be a team player, willing to share best practices with colleagues as well as learn from other’s successes.
Hardworking and organized; plan creative activities and communicate with compassion.
Assertive communication skills with good boundaries and stress management.
Proficient in Word, Excel, Outlook, and can learn data entry systems.

Agency Requirements

Non-smoking worksite: no tobacco products allowed.
Must be at least 21 years old.
Must pass extensive post-offer, pre-employment background check and drug test.
If in recovery, must have at least 2+ years of continuous sobriety.
Must be able to lift up to 40 pounds and climb stairs.
Must have own reliable transportation, current valid driver’s license, clean driving recording, and willing to transport clients in own car or agency vehicles on occasion.
Must be able to adhere to ethical standards and maintain confidentiality.
Must have a compassionate and empathetic approach to working with individuals facing substance use and mental health challenges.

Education Required

Bachelor’s degree and willing to work toward Substance Use Disorder Counselor (SUDC) or Advanced SUDC (ASUDC) or Social Service Worker (SSW) licensure.

Hours and Schedule

Full-time: Monday through Friday, 8:30 a.m. to 5 p.m. with occasional evening hours to support client family visits. On-site in Salt Lake City

Wages & Benefits

$20.00 to $24.00 per hour, based on experience and licensure. Full-time benefits package, including:

Health insurance (90% of base+ plan premium employer-paid);
Dental insurance (100% of employee premium employer paid);

Voluntary vision, short-term disability, and other ancillary insurance plans available;
30+ paid days off per year for holidays, vacation, sick time, and personal day;
