Social Services Supervisor

Posted Pay Range

$105,000 - $130,000 per year
Location: San Bernardino, California

About the Role

We are seeking a Social Services Supervisor to provide leadership, clinical oversight, and advanced guidance for the delivery of psychosocial services within the interdisciplinary team (IDT). This position supervises Master’s-level Social Workers (MSWs) and Social Worker Assistants (SWAs), ensuring high-quality participant care, regulatory compliance, accurate documentation, and effective coordination of services.

The Social Services Supervisor plays a key role in supporting participant safety, aging in place, social determinants of health (SDOH), housing stability, and successful transitions of care. The position provides consultation on complex cases, oversees grievances and service delivery requests, supports incident investigations, and serves as a liaison among the IDT, Center Triad, behavioral health and clinical leaders, and community partners.

The Supervisor will maintain a caseload proportional to census and staffing, with time spent across responsibilities adjusted according to operational needs and leadership direction.

Key Responsibilities

Leadership, Supervision & Clinical Oversight

Provide day-to-day supervision, coaching, and performance management for MSWs and SWAs.
Ensure clinical quality, timely documentation, and compliance with regulatory and organizational requirements.
Review assessments, care plans, service delivery requests, grievances, incident documentation, and related work on a scheduled basis.
Monitor caseloads, productivity, KPIs, workload distribution, and staffing coverage.
Lead onboarding, training, competency development, and ongoing professional growth.
Interview, train, assign work, conduct performance reviews, coach employees, recognize performance, and address performance concerns.
Foster a positive, collaborative team culture through regular one-on-one and team meetings.

Regulatory, Process & Quality Compliance

Ensure timely completion of required PACE psychosocial assessments and care plans.
Oversee grievance, service determination request, appeal, and root-cause analysis (RCA) processes.
Ensure compliance with CMS, state regulations, and organizational policies and procedures.
Lead or support quality improvement initiatives, audits, and case reviews.

Participant Care & Complex Case Consultation

Provide advanced consultation on high-risk and complex cases involving family dynamics, behavioral health, substance use, housing crises, and other psychosocial concerns.
Support escalation and response to safety concerns, including abuse, neglect, and mandated reporting.
Facilitate collaboration among medical, behavioral health, eligibility, operational, and social services teams.
Lead or participate in care conferences, family meetings, and complex transition-of-care planning.

Health, Safety, Housing & SDOH

Oversee housing-related processes, including facility placement, level-of-care changes, tenancy disruptions, rehousing needs, and safety interventions.
Coordinate with facilities, Adult Protective Services (APS), ombudsman offices, and community organizations.
Guide staff in connecting participants with benefits, community resources, and SDOH supports.
Help ensure participant safety, risk mitigation, and appropriate access to services.

Communication & Documentation

Serve as a communication hub for escalated social services matters involving caregivers, IDT members, Center leadership, and external partners.
Ensure accurate and timely documentation in electronic medical records (EMRs) and required systems.
Maintain confidentiality and compliance with organizational policies.

Administrative Leadership

Assist leadership with implementation and review of Social Work policies, procedures, and workflows.
Delegate appropriate logistical responsibilities to SWAs to support MSWs, comfort care coordination, and behavioral health services.
Partner with state and behavioral health leadership to implement best practices and standardized processes.
Conduct case record reviews to ensure adherence to agency and state requirements.
Monitor and evaluate service delivery within the assigned catchment area.
Interview participants to address special concerns and appropriately escalate needs.
Participate in committees and meetings, including Social Work Cross-Site meetings, as assigned.
Perform other duties as assigned.
Fulfill applicable responsibilities outlined in the MSW job description based on caseload requirements.

Key Performance Indicators

Success in this role will be measured through:

Regulatory and documentation compliance, including timely assessments, care plans, service requests, grievances, and incident documentation.
Staff performance against established benchmarks and KPIs.
Housing, safety, and SDOH outcomes, including mitigation of high-risk and unresolved concerns.
Effective IDT integration and participant experience.
Team engagement, retention, professional development, and completion of performance reviews.
Quality, timeliness, and completeness of the Supervisor’s own caseload documentation.
Consistent modeling of trauma-informed and person-centered care.

Travel Requirements

Travel may be required.
Overnight travel may include local or out-of-state travel.

Required Qualifications

Education

Master’s degree from a school of social work accredited by the Council on Social Work Education (CSWE).

Licensure & Certifications

Minimum state certification or registration appropriate for a master’s-level social worker, such as LSW.
New Mexico: Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) through the New Mexico Board of Social Work.
Meet all state requirements for practicing social work in healthcare settings, including supervision requirements where applicable.
Current state-issued driver’s license, good driving record, personal transportation, and automobile insurance as required by law.
CPR/BLS and First Aid certification as required by state regulations.

Experience

At least 2 years of experience supervising staff.
At least 3 years of social work experience in a medical or healthcare setting, including assessments, care planning, and case management.
At least 1 year of experience working with complex geriatric or adult populations with multiple medical and psychosocial conditions.
At least 1 year of experience addressing health and safety concerns, housing instability, and social determinants of health.
At least 2 years of experience conducting psychosocial and risk assessments and developing care plans.
Knowledge of state, county, and local governmental and community-based resources.
Extensive knowledge of Medicare, Medicaid/Medi-Cal, and services available to dually eligible participants.
Familiarity with medical terminology, common medical conditions and medications, and the needs of older adults with complex, multimorbid conditions.

Preferred Qualifications

5+ years of experience in Social Services.
3+ years working with complex geriatric/adult populations with multimorbid medical and psychosocial conditions.
3+ years of supervisory experience.
Experience working in an interdisciplinary team (IDT) environment.
Bilingual skills.

Compensation

Posted Pay Range: $105,000 - $130,000 per year
