Position Summary
The Lead Care Manager & Clinical Supervisor is a senior clinical leader responsible for establishing clinical guardrails, supervising the care management team, and maintaining an active caseload of clients. This role combines direct client care with clinical leadership, ensuring care delivery remains evidence-based, ethical, and aligned with the highest professional standards while modeling excellence in care management practice.
This position serves as the strategic bridge between executive leadership and frontline clinical operations, assuming responsibility for both organizational clinical oversight and hands-on care management. The Lead Care Manager will balance high-quality client care, staff development, operational efficiency, and financial sustainability to support VCC's continued growth.
The mission of this role is to navigate the complex balance between clinical excellence, staff wellbeing, operational efficiency, and sustainable business practices. The Lead Care Manager partners closely with the CEO to ensure exceptional client outcomes while developing a highly engaged and accountable clinical team.
Direct Client Care & Field Work
In addition to providing clinical leadership and supervision, the Lead Care Manager & Clinical Supervisor maintains an active caseload and provides direct care management services to assigned clients. Remaining actively engaged in client care allows the Lead Care Manager to model best practices, support organizational capacity, mentor clinicians through real-world clinical situations, and maintain strong clinical competency.
Responsibilities include:
- Maintain an active caseload of clients while balancing leadership and supervisory responsibilities.
- Conduct in-home visits, hospital visits, assisted living and skilled nursing facility visits, and community-based client meetings as appropriate.
- Complete comprehensive biopsychosocial assessments and develop individualized care plans.
- Coordinate care with physicians, hospitals, home health agencies, hospice providers, community resources, attorneys, fiduciaries, and other interdisciplinary professionals.
- Provide ongoing care coordination, advocacy, education, and crisis intervention for clients and families.
- Respond to urgent client situations while ensuring timely follow-up and communication.
- Complete accurate, timely, and professional documentation in accordance with VCC standards.
- Serve as the lead clinician on high-complexity cases and provide consultation to other care managers.
- Model VCC's standards of compassionate, relationship-centered care through direct client interactions.
Clinical Leadership & Risk ManagementClinical Case Conferences
Lead weekly clinical case conferences to review complex client situations, identify risks, provide clinical consultation, and develop collaborative care planning strategies.
Clinical Governance
Establish, implement, and enforce evidence-based clinical standards, documentation expectations, and best practices across the clinical team.
Risk Mitigation
Proactively identify clinical risks, develop crisis intervention strategies, and collaborate directly with the CEO regarding significant client safety concerns, ethical issues, and professional standards.
Clinical Decision Making
Evaluate competing priorities while balancing client safety, organizational resources, financial sustainability, and clinician capacity when accepting and managing high-complexity clients.
Caseload Sustainability & Strategic Staffing
The Lead Care Manager utilizes data-driven leadership to maintain healthy clinician workloads while ensuring organizational sustainability.
Responsibilities include:
- Monitor clinician utilization, productivity, and billable hours.
- Analyze staffing capacity and workload distribution.
- Ensure clinicians meet established utilization expectations.
- Monitor client assignment balance and workload equity.
- Advise the CEO regarding staffing needs, hiring priorities, and organizational growth.
- Evaluate operational trends to support long-term financial health.
Clinical Supervision & Performance Management
The Lead Care Manager oversees the complete supervisory lifecycle of clinical staff.
Onboarding
- Lead orientation and onboarding of new clinicians.
- Mentor new employees during their transition into the organization.
- Ensure consistency with VCC standards from day one.
Clinical Supervision
- Conduct regular individual supervision meetings.
- Provide coaching, mentorship, and professional guidance.
- Support clinicians in navigating complex family dynamics and ethical situations.
Performance Management
- Complete quarterly performance evaluations.
- Conduct comprehensive annual reviews.
- Address performance concerns through coaching and development plans.
- Recognize outstanding performance and professional growth.
Professional Development
- Develop individualized growth plans.
- Identify educational opportunities.
- Foster leadership development within the clinical team.
AI Clinical Governance & DocumentationEthical AI Oversight
Provide oversight of AI-assisted tools used within care management while ensuring technology enhances—not replaces—professional clinical judgment.
Documentation Audits

Brand Voice

Organizational Leadership
- Partner closely with the CEO regarding clinical operations and strategic initiatives.
- Promote a culture of accountability, collaboration, and continuous improvement.
- Assist with developing and refining policies, workflows, and clinical processes.
- Support organizational growth through operational leadership and innovation.
- Represent VCC professionally within the healthcare and aging services community.
Required Qualifications
- Bachelor's degree in Social Work, Nursing, Human Services, Gerontology, or a related healthcare field.
- Minimum five (5) years of clinical care management or healthcare experience.
- Demonstrated leadership, mentoring, and supervisory experience.
- Experience managing complex geriatric or medically complex cases.
- Ability to travel throughout San Diego County for client visits.

Pay: $55.00 - $85.00 per hour
