Full Job Description

About the Role

Solara Mental Health is a leading behavioral health organization in San Diego providing residential, PHP, IOP, and detox services. We are looking for a dedicated and organized Case Manager to join our team Monday through Friday. This role is at the heart of our day-to-day client experience ,you will be one of the first faces clients see each morning, a key coordinator of their care, and a reliable support for their transition back into the community.

Schedule & Compensation

Monday through Friday, 9:00 AM – 5:30 PM
Starting pay: $25.00 per hour
Full-time, non-exempt position

What Your Day Looks Like

Each morning at 9:00 AM, you will begin your shift by going into the milieu to check in with clients face-to-face: greeting them, assessing how they are doing, and helping support their Activities of Daily Living (ADLs) as they start their day. From there, your day moves through a combination of case management, coordination, transportation, and office operations.

Essential Functions

Client & Milieu Support

Arrive at 9:00 AM and begin each shift with a walk-through of the milieu to check in on clients, assess their mood and needs, and support morning ADLs
Build rapport with clients and serve as a consistent, trusted point of contact throughout their time in the program
Monitor client wellbeing and escalate concerns to the clinical team as appropriate

Case Management

Conduct initial and ongoing comprehensive assessments of clients' mental, physical, and psychosocial needs
Develop and implement individualized case management plans in collaboration with clients, families, and the clinical team
Monitor and evaluate the effectiveness of care plans against established health and wellness goals
Facilitate group and individual sessions as needed, documenting all client interactions thoroughly
Coordinate care transitions and support clients through each phase of treatment toward safe community reintegration
Communicate regularly with client families as a point of contact for updates and coordination

Transportation & Offsite Appointments

Provide transportation for clients to and from offsite medical, psychiatric, and community appointments
Ensure clients arrive on time and are prepared for appointments; accompany clients when appropriate
Maintain a valid California driver's license with a clean driving record; operate company vehicles safely and responsibly
Document all transportation activities and appointment outcomes in the client record

Office Management

Coordinate client appointments and program schedules, ensuring adherence to treatment protocols
Maintain organized documentation and ensure compliance with healthcare regulations
Manage and maintain office operations including overseeing the reception area and ensuring a welcoming environment for clients and families
Support the clinical team with administrative tasks as needed

Qualifications

Education & Experience

Bachelor's degree in Social Work, Psychology, or a related field preferred
Minimum of one year of experience in a case management or related role
Experience working in a residential, PHP, IOP, or behavioral health setting a plus

Certifications & Credentials

Current CPR and First Aid certification required
Valid California driver's license with a clean driving record required
Current TB test clearance required

Skills

Strong interpersonal and communication skills — written and verbal
High proficiency in Google Suite, Electronic Health Records (EHR), and general computer use
Ability to manage stress, adapt quickly, and prioritize effectively in a fast-paced environment
Detailed knowledge of federal and state regulations (e.g., FMLA, SSI) as applied to treatment and documentation
Experience with biopsychosocial assessments, dual diagnosis, and non-pharmacological symptom management preferred

Physical Requirements

Regular requirement to talk, hear, stand, walk, and bend throughout the shift
Must be able to safely operate a motor vehicle
Occasionally required to lift items over 20 pounds
