JOB SUMMARY
HealthRIGHT 360, a nonprofit organization and a family of programs, is committed to providing accessible and comprehensive healthcare services to vulnerable populations. Our mission is to tackle systemic barriers to healthcare and promote health equity for all. We offer a wide range of services, including mental health care, residential and outpatient substance use treatment, and primary health services. Additionally, we provide transitional support for individuals re-entering the community after involvement in the criminal justice system. By integrating physical and behavioral health, we empower individuals to overcome challenges by addressing social determinants of health, fostering resilience, and facilitating recovery.
The AARS program provides trauma-informed care designed to reduce the negative impacts of substance use. Participants collaborate closely with their counselors to create a personalized, strengths-based treatment plan tailored to meet their unique needs and goals. Services include individual and group therapy, case management, treatment planning, substance use counseling, crisis intervention, and referrals to social support services. In addition, the program offers prevention services such as outreach, health education, and peer support for the community.
The Family Specialist plays a vital role in supporting families facing complex challenges by providing strength-based, culturally responsive services that promote family stability, wellness, and resilience. This position works directly with youth, teachers, school staff, parents, caregivers and other collaterals in youth’s life, to assess needs, develop individualized service plans, and provide case management, such as connecting families to community resources and support. Through advocacy, and collaboration with multidisciplinary teams, the Family Specialist empowers families to achieve their goals, navigate systems, prevention and engagement services, develop youth resilience skills, and enhance parenting skills in a safe and supportive environment. This role requires a compassionate, organized, and collaborative professional who is committed to family-centered, trauma-informed care. Collaborate with multidisciplinary team and school staff to coordinate services for youth. Work Services are provided at the school, community, agency office, or any location that’s accessible to community served. Provide services to work at various school, community sites (Like Juvenile Hall), and youths of all ages and backgrounds.
Hours of operation are Mondays to Fridays from 7:00AM to 8:00pm. Some weekends and evenings on occasion for prevention or family services.
KEY RESPONSIBILITIES
Administrative Responsibilities
- Participate in case conferences, team meetings, and ca
- re-coordination efforts to ensure holistic, integrated support for the family unit.
Documentation Responsibilities
- Maintain accurate and timely documentation of family contacts, goals, service plans, and progress in accordance with agency and funder guidelines.
Community and Outreach
- Provide support and conduct regular check-ins to assess family well-being, reinforce goals, and build trust.
- Serve as a liaison between families and systems (schools, social services, mental health providers), ensuring family voices are heard and respected.
- Provide outreach and prevention services at school, clients’ homes, and other community sites such as Juvenile Hall.
Treatment / Direct Support Responsibilities
- Work collaboratively with families to identify strengths, needs, and goals, supporting them through individualized service planning.
- Encourage and support active family involvement in services, team meetings, treatment planning, and decision-making processes.
- Connect families to appropriate community resources such as housing, food assistance, education, employment services, and healthcare.
- Offer parenting education and coaching using evidence-informed tools to promote safety, nurturing, and developmentally appropriate care.
- Assist families in navigating crises and connect them with appropriate immediate support and resources as needed.
- Engage with families in a culturally sensitive and trauma-informed manner, respecting individual backgrounds, beliefs, and traditions.
Productivity and Coverage Responsibilities
- Must achieve service delivery goals as set forth by the program requirements.
- May be required to meet service delivery goals at other Santa Clara County Behavioral Health programs.
- Ensure coverage during absences or unplanned events by coordinating with team members and management.
- Adjusts workload as necessary to ensure uninterrupted service delivery, including stepping in to provide support during peak demand times or staff shortages.
- Adheres to the program’s scheduling protocols to maintain appropriate client-to-provider ratios.

QUALIFICATIONS
Education, Certification, or Licensure
- High School diploma or equivalent
- Valid CA Driver’s License.
- First Aid & CPR certification.
- Experience with system-involved families (e.g., child welfare, probation, behavioral health) is highly desirable.
Desired:
- Bachelors degree in a related field.
Knowledge and Skills
- Familiarity with trauma-informed, strength-based, and culturally responsive practices.
- Understanding community resources, family dynamics, and child development.
- Strong documentation and case note writing skills.
- Ability to maintain accurate records and comply with confidentiality standards (e.g., HIPAA).
- Empathy, adaptability, strong interpersonal communication, and a nonjudgmental approach.
