Job Title/Position: Intake Coordinator

Reports To: Executive Director/Administrator or Designee

Job Description Summary
Responsible for assisting in managing all aspects of the patient intake process including assisting in managing all the members of the intake team, establishing and maintaining positive relationships with customers and referral sources, responding to customer requests and concerns, and managing the insurance verification and authorization processes.

Essential Job Functions/Responsibilities

Receives and processes all requests for services.
Completes referral form with medical and demographic information.
Distributes referrals for assignment, scheduling, and billing.
Disseminates information and answers inquiries related to the availability of resources.
Establishes system for prior authorization from third-party payers.
Maintains logs and files necessary to assure compliance.
Develops and implements procedures for completing and filing reimbursement forms.
Reviews all communication from insurance companies/other payers.
Communicates with families and clients regarding the coverage for home care services. Acts as an educational resource to physicians, social workers, discharge planners, and utilization review staff regarding home health services and available coverage.
Collaborates with Marketing Staff in providing education and resources to Agency staff and community.
Educates clinical and accounting staff about insurance benefits and coverage criteria.
Works with community referral sources, families, and Agency staffs to problem solve and identify services available to the client either through Agency services or the community.
Participates in Agency orientation for new staff in matters pertinent to client admission criteria.
Provides outside facilities, if indicated, with information about availability and type of services and reimbursement.
Follows up on client transition to home. Communicates with referral sources.
Demonstrates teamwork and effective communication to accomplish Agency and client goals.
Participates in care conferences and other team/Agency meetings.
Shares client observations and pertinent information regarding reimbursement.
Initiates interdisciplinary referrals and works with teams to identify and access outside resources as appropriate.
Participates in Agency quality improvement activities. Initiates activities related to improvement activities in admission/intake area.
Performs job in compliance with Agency policies and procedures and professional and community standards.
Accepts responsibility in accordance with the role of Intake Coordinator/Support.
Provides services in accordance with state and federal laws, and CHAP standards.
Accepts responsibility for personal and professional development and identified learning/developmental needs.
Maintains confidentiality in all aspects of the job.
Performs other duties and activities as deemed necessary.

Position Qualifications

High School Diploma
Knowledge of medical terminology
Information systems knowledge
Demonstrates ability to be supervised by Clinical Nursing Supervisors
Demonstrates good communication and public relations skills.
Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities.
