Position Overview
Reporting to the Housing and Treatment Director, the DeVaul Ranch Case Manager provides intensive, person‑centered case management and housing stabilization services to approximately 25 adult residents of DeVaul Ranch who are living in substandard or unsafe conditions and are at high risk of homelessness. The position serves as a bridge from current CRG oversight to Restorative Partners’ long‑term ownership and stewardship of the site, ensuring residents are not left without resources during the transition. The Case Manager focuses on building confidence, fostering independence, and equipping residents with the tools needed for long‑term self‑sufficiency through skill‑building, employment readiness, and community engagement. The Case Manager models engagement, flexibility, accountability, and a strong sense of community while maintaining clear communication, empathy, and alignment with Restorative Partners’ mission and values.
Key Roles & Responsibilities
- Conduct regular outreach to all DeVaul Ranch residents, including those who are disengaged, working off‑site, or minimally interactive, using trauma‑informed and strengths‑based approaches.
- Build trusting relationships with residents who may be ambivalent or resistant to services, emphasizing choice, dignity, and respect.
- Complete holistic intake and ongoing assessments (housing, income, health, behavioral health, legal, social supports, safety risks) and document in the agency’s case management system and/or HMIS.
- Co‑create individualized service and housing stability plans with residents that include concrete, time‑bound goals related to housing safety, benefits access, health, life skills, and employment/readiness.
- Coordinate with Restorative Partners leadership, CRG, property owners, and county partners to address urgent basic needs (e.g., potable water access, habitability and safety issues, relocation from unsafe structures) within project and funding parameters.
- Support residents in navigating landlord/tenant issues, reasonable accommodations, and preparation for any future relocation as the property transitions to a Healing and Restoration Campus model.
- Assist residents with enrollment and re‑enrollment in public benefits (CalFresh, Medi‑Cal, SSI/SSDI, General Assistance) and coordinate with CalAIM and other care management providers as appropriate.
- Facilitate referrals and warm handoffs to primary care, mental health, SUD treatment, and other community services, and support follow‑through with appointments and care plans.
- In collaboration with the Program Coordinator, provide or coordinate life skills coaching (budgeting, housekeeping, cooking, transportation planning, communication and conflict resolution) to build resident confidence and independence.
- In collaboration with the Program Coordinator, provide employment readiness support (résumé development, job search, interview preparation, basic work‑readiness coaching) and coordinate with workforce, training, and social enterprise partners.
- Encourage and support resident participation in pro‑social, restorative, and faith‑informed activities (support groups, circles, volunteer/service, peer leadership) aligned with Restorative Partners’ mission.
- Foster a sense of community at the Ranch through resident meetings, restorative dialogues around shared space and safety, and connection to local neighborhood resources.
- Maintain timely, accurate, and confidential case notes, service plans, and outcome data in case management and/or HMIS systems in alignment with reporting and compliance expectations.
- Track outcomes such as housing stability, income/benefits gained, connection to health/behavioral health services, employment/education engagement, and reduced justice involvement to demonstrate impact and inform program improvements.
- Participate in case conferencing with county and community partners and internal Restorative Partners staff to coordinate services and ensure continuity of care.
- Provide regular updates to leadership on trends, gaps, and resident needs to guide development of the long‑term Healing and Restoration Campus model at DeVaul.
Qualifications and Requirements
- Bachelor’s degree in social work, psychology, human services, or related field, or equivalent combination of education and relevant experience with individuals experiencing homelessness, serious behavioral health conditions, or justice involvement.
- Minimum 2–3 years of case management, care coordination, outreach, or housing navigation experience, preferably within homeless services, behavioral health, or reentry contexts.
- Demonstrated ability to engage hard‑to‑reach adults, including individuals who are resistant to services, working off‑site, or living in non‑standard/unsafe housing conditions.
- Willingness and ability to work with the sex offender population.
- Familiarity with Housing First principles, harm reduction, trauma‑informed care, and recovery‑oriented, strengths‑based case management.
- Experience with documentation, data collection, and use of electronic case management or HMIS systems; ability to meet grant reporting and compliance standards.
Supervision, KPIs, and Workload
- Receives weekly individual supervision and participates in team case conferences focused on quality and outcomes.
- Key performance indicators include:
- Percent of DeVaul residents with completed assessments and individualized service plans within 30 days of engagement.
- Number and percent of residents with improved habitability/safety or successful transition to more stable housing or bridge housing.
- Number and percent of residents connected to or maintaining public benefits, health/behavioral health care, and/or employment or training.
- Reduction, where trackable, in ER use, law enforcement contacts, or justice involvement in collaboration with county partners.
- Expected caseload: approximately 25 residents, with service intensity adjusted based on acuity and engagement level.
Knowledge, Skills, Talents, & Abilities
- Certification or strong competency in motivational interviewing (or willingness to obtain).
- Understanding of restorative justice principles and practices and familiarity with criminal justice system processes and terminology.
- Knowledge of local employment markets and community resources.
- Strong communication, interpersonal, and active listening skills, with an empathetic and trauma‑informed approach.
- Ability to work effectively with diverse populations and maintain professional boundaries and confidentiality.
- Creative problem‑solving, adaptability, and comfort working in a dynamic, transitional environment.
- Proficiency with case management and database systems.
Essential Requirements
- Current driver’s license, valid auto insurance, and reliable transportation.
- Successful completion of a comprehensive background check.
- Bachelor’s degree in Social Work, Psychology, Criminal Justice, or related field preferred.
- Minimum of 2 years of experience in workforce development, housing, behavioral health, or related field preferred.
- Proficiency in Google Suite, Microsoft Office, and database management systems.
- Ability to work flexible hours, including occasional evenings and weekends.
- Fluency in English; bilingual English/Spanish highly desirable.
Physical Demands
- Ability to sit for extended periods, with regular standing, walking, and light lifting (up to 25 pounds).
- Frequent use of computer, phone, and standard office equipment; substantial reading, writing, and note‑taking.
- Work is primarily based at DeVaul Ranch, with some community‑based and office‑based activities; requires navigating uneven terrain and non‑traditional structures safely.
- The DeVaul Ranch is in transition from Sunny Acres operations to the Healing and Restoration Campus (HARC) operated by Restorative Partners; staff must be adaptable to evolving systems and practices.
- During construction, some work may occur at outdoor tables and chairs rather than in a traditional office setting.
