POSITION SUMMARY:

Case Managers in the Permanent Supportive Housing (PSH) Department of Episcopal Community Services serve as integral members of the Support Services team at designated housing sites. They provide personalized case management and support services to formerly homeless adults residing in these locations. Responsibilities encompass clinical needs assessments, service planning, supportive counseling, and consultations with residents. Additionally, Case Managers facilitate community program and benefit referrals, organize support groups, initiate community-building activities, and undertake other essential case management duties.

PRIMARY DUTIES AND RESPONSIBILITIES:

1. Conduct intake assessments for assigned residents on move-in day, covering health status, psychosocial history, financial status, substance use history, and housing background.

2. Regularly engage with assigned caseload residents and respond promptly to incidents or signs of crisis, prioritizing direct, in-person interactions.

3. Collaborate with residents to create and implement personalized Service Plans aligned with their goals and preferences.

4. Provide referrals to in-house or external resources on substance use, vocational training, legal aid, health services, housing support, financial management, etc., when on-site resources are insufficient.

5. Assess residents' eligibility for benefits and assist with enrollment in relevant programs.

6. Offer support and guidance on budgeting, including the development of voluntary money management plans.

7. Advocate for residents with Property Management to address issues jeopardizing their housing stability, aiming to prevent evictions where feasible.

8. Mediate conflicts among residents or staff to foster a conducive living environment promoting respect, cooperation, self-sufficiency, and tenant empowerment.

9. Conduct Wellbeing Checks twice a week and Emergency Safety Checks as needed to ensure resident safety.

10. Facilitate at least one weekly therapeutic or psycho-educational group and assist with organizing other program events and community-building activities.

11. Maintain accurate, confidential, and professional documentation of services provided in resident files, meeting all administrative requirements within specified deadlines.

12. Assist residents with planned exits from the program by facilitating comprehensive discharge planning and making necessary referrals for ongoing support services.

13. Contribute to the development and implementation of new programs, groups, and activities under the guidance of the Support Services Manager.

14. Provide backup coverage for other case managers as directed.

15. Share relevant information and resources with residents and staff.

16. Adhere to all applicable policies and procedures of Episcopal Community Services.

17. Cultivate positive working relationships with ECS employees, volunteers, residents, and external agencies.

18. Attend all assigned meetings and fulfill any other duties as directed.

Qualifications

REQUIRED QUALIFICATIONS:

Case Manager ($28.00/hr.): High School Diploma + 4 years experience or AA degree in similar field + 3 years experience, or CHW Certificate/CADC + 2 4 years experience, or BA/BS degree in similar field + 2 4 years experience, or MA/MS degree in similar field + 1 year experience.
Senior Case Manager ($30/hr.): MA/MS + at least 2 years experience, or BA/BS + at least 4 years experience, CHW Certificate/CADC + 4 years experience.

PLUS:

Knowledge of local social services and other community resources for underserved populations and severely low-income individuals and families.
Must show patience, compassion, and empathy to the participants that we serve.
Must possess excellent charting, recordkeeping, and professional writing skills for daily documentation.
Must be able to quickly assess and respond to a variety of situations, including medical emergencies, psychiatric emergencies, conflict resolution, de-escalation interventions, as well as other types of crises.
Computer skills and moderate experience using Microsoft Outlook, Word, and other MS Office applications is required.
Strong interpersonal skills, a team approach, and the ability to communicate effectively both orally and in writing are essential.
Demonstrated cultural competency in working with diverse, underserved, and and the ability to communicate in Spanish is desirable. Bi-lingual positions (if open) pay a 5% premium!

Great collaborator with the ability to maintain professional, respectful, honest, and calm interactions with residents, staff, and outside partners is required.
Be able to work as part of a multi-disciplinary team.
Must be comfortable facilitating groups and hosting events and activities.

OTHER

Must secure finger image screening, annual TB screening.
Must be able to meet physical requirements of the position which may include long periods at a desk and/or computer workstation, some travel to various sites, and lifting of up to 20 lbs.
