HEALTH NAVIGATOR JOB DESCRIPTION
Position Type: Full TimeMonday-Friday: 9:00AM-5:00PM*
*Generally Monday-Friday, 9:00 a.m.-5:00 p.m., with 2-3 evening programs and approximately one weekend event per month to support participant programming, workshops, and community events.

POSITION DESCRIPTION
The Health Navigator serves as Dress for Success Oregon's subject matter expert on health navigation and participant wellness. This position provides individualized support to women and gender expansive individuals facing barriers to employment, self-sufficiency, and overall well-being.
Working closely with participants, community partners, and DFSO staff, the Health Navigator connects individuals to healthcare, behavioral health, public benefits, housing, childcare, transportation, food assistance, and other critical supports that promote long-term stability and employment success.
The Health Navigator will play a key leadership role within WorkWell, helping expand DFSO's holistic workforce development model by integrating health and wellness into participants' employment journeys.
This position does not provide medical diagnosis, clinical treatment, therapy, or legal advice. The Health Navigator provides education, resource coordination, advocacy, and referrals while working within the scope of a non-clinical navigator role.
WHY THIS ROLE MATTERS
The Health Navigator is a new role that will help DFSO expand its holistic workforce development model by ensuring participants can access the health, wellness, and basic needs supports necessary to succeed in employment and life. This position plays a critical role in reducing barriers and connecting participants to resources that promote long-term stability and economic mobility.
KEY RESPONSIBILITIES
Program Development & Leadership
- Support the implementation and growth of DFSO's Health Navigation program in partnership with the Executive Director.
- Help refine policies, procedures, referral pathways, and participant workflows to support effective service delivery.
- Contribute to ongoing improvements in participant intake, documentation, and follow-up processes using a trauma-informed approach.
- Identify opportunities for innovation and continuous improvement while helping shape the future of WorkWell's holistic workforce development model.

Participant Support & Navigation
- Conduct participant assessments and develop individualized action plans.
- Connect participants to healthcare, behavioral health, housing, childcare, transportation, food assistance, and other community resources.
- Provide ongoing advocacy, referrals, care coordination, and follow-up support.
- Assist participants with accessing and maintaining public benefits, including Oregon Health Plan (OHP).
- Maintain accurate participant records and documentation.
Health & Wellness Programming
- Lead the development of DFSO's health and wellness strategy, including workshops, support groups, affinity spaces, partnerships, and participant resources.
- Partner with the Program Facilitator to develop and deliver health and wellness workshops, with the Health Navigator serving as the lead on health-related curriculum and content.
- Create, facilitate, and foster peer support groups for women and gender expansive individuals, including groups focused on domestic violence survivors, postpartum support, perimenopause, caregiving, gender equity, LGBTQIA+ inclusion, and other topics that promote connection, healing, and resilience.
- Connect participants to primary care, reproductive healthcare, behavioral health providers, postpartum support, and gender-affirming healthcare services.
- Develop relationships with healthcare providers and community organizations to expand participant resources and educational opportunities.
Partnerships & Community Engagement
- Build and maintain partnerships with healthcare providers, Coordinated Care Organizations (CCOs), community-based organizations, and government agencies.
- Coordinate referrals, health events, workshops, and resource opportunities that support participant
- success.
- Represent Dress for Success Oregon in community collaborations and partnership meetings.
Staff Support & Collaboration
- Serve as DFSO's subject matter expert on health navigation and participant wellness.
- Train and support staff on trauma-informed referral practices, participant boundaries, and available community resources.
- Collaborate with program staff to provide coordinated, holistic participant support.
Data & Program Evaluation
- Track participant outcomes, referrals, and service utilization.
- Support grant reporting, program evaluation, and continuous quality improvement.
- Ensure participant information is maintained confidentially and in accordance with organizational policies.
EDUCATION & EXPERIENCE
- Bachelor's degree in Social Work, Public Health, Human Services, Community Health, Counseling, or related field preferred.
- Minimum of two (2) years of professional experience providing direct services through health navigation, care coordination, health-related resource referral, case management, public health, community health, social work, or a related health or human services field.
- Knowledge of Oregon Health Plan (OHP), Oregon Health Authority (OHA), CareOregon, and Oregon's healthcare landscape.
- Experience working with individuals facing multiple barriers to employment and self-sufficiency.
- Strong communication, organizational, and relationship-building skills.
- Experience with Salesforce or similar CRM platforms preferred.
- Bilingual or multilingual skills are a plus.
- Experience supporting survivors of domestic violence, LGBTQ+ communities, BIPOC communities, or justice-involved individuals is highly valued.
- Knowledge of trauma-informed, participant-centered, and culturally responsive service delivery.
COMPENSATION
Wage: Salary, $60,000-$65,000 annual

PTO: 120 hours (annual) not including 10 paid holidays and 1 holiday break between Christmas Eve to New Years Day
