We are seeking a highly organized, patient centered, and proactive Patient Care Coordinator to support operations across our wound care locations. This is a dynamic and fast paced role that requires strong communication, critical thinking, emotional intelligence, and attention to detail.

The ideal candidate is dependable, adaptable, solution oriented, and thrives in a high responsibility healthcare environment where patient experience, operational efficiency, and teamwork are essential.

Position Summary

The Patient Care Coordinator serves as a central point of communication between patients, families, providers, clinical teams, and administrative staff. This role is responsible for coordinating patient care services, supporting daily clinic operations, assisting field staff and providers, and ensuring patients receive timely, organized, and compassionate care throughout their treatment journey.

This position supports multiple wound care locations and requires the ability to manage multiple priorities while maintaining professionalism, urgency, and exceptional customer service.

Key Responsibilities

Serve as a primary point of contact for patients and families throughout the care process
Coordinate patient care services across multiple wound care locations
Facilitate communication between providers, clinical teams, referral sources, patients, families, and external healthcare partners
Schedule and coordinate patient appointments, follow ups, procedures, referrals, and treatments in a timely and organized manner
Monitor progression of patient care plans and assist in removing barriers to treatment
Support field staff and providers with coordination, communication, and operational needs
Gather and maintain accurate patient information, documentation, records, and required forms
Ensure complete and accurate patient registration, insurance verification, authorization tracking, and demographic updates
Contact insurance companies to ensure approval requirements are met and provide necessary medical information to support reimbursement processes
Maintain HIPAA compliance and patient confidentiality standards at all times
Conduct outreach regarding upcoming appointments, missed visits, follow ups, and patient care needs
Assist with staff scheduling and operational coordination as needed
Conduct regular communication and collaboration with the interdisciplinary care team
Participate in quality assurance initiatives, workflow improvement efforts, and service delivery monitoring
Collaborate closely with interdisciplinary teams to ensure continuity of care and positive patient outcomes
Educate patients and families regarding care plans, appointments, medications, and treatment expectations
Assist in resolving patient concerns, scheduling conflicts, and coordination issues professionally and efficiently
Track patient information, files, tasks, forms, and care coordination activities accurately and timely
Evaluate outcomes of care with the interdisciplinary team by monitoring coordination effectiveness and patient progression
Establish and maintain professional relationships with healthcare partners and service providers
Maintain professionalism, compassion, and urgency in all patient interactions
Support overall clinic operations and team collaboration efforts as needed

Qualifications & Requirements

Previous experience in a medical office, healthcare coordination, scheduling, or administrative support role preferred
Experience in wound care, specialty care, home health, or multi-site healthcare operations is a plus
Familiarity with medical terminology and healthcare workflows required
Experience with electronic health record systems and office software preferred
Knowledge of insurance verification, prior authorizations, and coordination of care processes preferred
Highly organized with the ability to manage multiple priorities in a fast paced environment
Strong written and verbal communication skills
Ability to work independently, exercise creativity, and maintain a positive attitude
Strong problem solving abilities and resourcefulness
Strong understanding of HIPAA, patient confidentiality, and professional healthcare communication standards
Ability to maintain confidentiality and handle sensitive patient information with professionalism and discretion
Ability to multitask effectively while maintaining accuracy and efficiency
Commitment to providing exceptional patient care and customer service
Ability to collaborate effectively with providers, clinical teams, and administrative staff
