Full Job Description

The Advanced Practice Provider II (APP) is responsible for providing direct patient care to assigned patients within Cottage Medical Group (CMG) and/or the hospital. Functions independently and in collaboration with the supervising clinic physician(s) and leadership in the delivery of patient care. Assesses, interprets, and integrates findings to determine appropriate diagnostic, educational and therapeutic management plans for patients.

Typical work scenarios require specialized knowledge, critical thinking, good judgment based upon the principles of biological, physiological, behavioral, and sociological sciences skills, as well as strong customer service skills. Internal contacts include ancillary services departments, clinic and/or hospital staff, supervising physician(s) as well as other physicians, and management throughout CMG and/or the hospital. External contacts include patients, physicians, outside medical staff, and community resource services.

MAJOR ACCOUNTABILITIES / SPECIFIC JOB COMPETENCIES

This is not an exhaustive statement of duties, responsibilities, or requirements. Employees will be required to perform any job, with related instruction given by their supervisor, subject to reasonable accommodation.

Performs medical services in both clinic and hospital settings within the scope of education, training, and experience including: conducting patient visits per accepted practice standards, obtaining patient histories and performing physical examinations, ordering and evaluating appropriate laboratory and diagnostic studies, developing and implementing treatment plans in accordance with clinical practice guidelines and protocols, monitoring the effectiveness of therapeutic interventions, and assessing and managing patient follow-up care. This role involves either working primarily in a clinic setting with regular hospital rounds, or working exclusively in a hospital environment, depending on organizational needs and job assignment. (40%)
Collaborates with physicians in managing acute and long-term medical needs of patients. Duties include consultations, monitoring and continuity of care between clinic visits through pre- and post-treatment surveillance and post-op visits in CMG clinic(s) and/or hospital. Maintains and reviews patient records, charts and other pertinent information. Posts tests and examination results. Triages patient telephone calls and provides consultation. Seeks and obtains input from physicians regarding questions and issues pertaining to the treatment and/or care of patients as needed. Works in collaboration with physicians and the care treatment team in the establishment and, management of Transitional Care and Chronic Care Management. Manages own sub-clinic to provide patient assessment, education and clinical recommendations within scope of practice. (30%)
Prescribes pharmaceuticals, other medications, and treatment regimens as appropriate to assessed medical conditions and refers patients to specialist and to relevant patient care options as appropriate. Performs Peripheral intravenous access consistent with training guidelines, utilizing appropriate techniques and devices. (10%)
Demonstrates good organizational and communication skills in coordinating services with co-workers and other departments/services. Responds to stressful situations in a calm and rational manner, and cooperates with all to facilitate integration of services. Behaves in a professional and empathetic manner to build trust, comfort, understanding, and relationships with patients, clinic management and personnel. (10%)
Facilitates the education and professional development of patient care practitioners. Participates in the education of clients, families, and the community. Assists staff with the maintenance of clinical competencies and the promotion of critical thinking skills. Develops the framework for clinical orientation of professional staff in conjunction with unit-based educators, preceptors, and directors; evaluates efficacy of the program. Mentors preceptors and other professional nursing staff in unit-based leadership roles. May be required to supervise ancillary staff and perform research. (10%)

QUALIFICATIONS - All job qualifications listed indicate the minimum level necessary to perform this job proficiently.

LEVEL OF EDUCATION

PHYSICIAN ASSISTANT - Graduate of an accredited Physician Assistant program, as defined by the California Physician Assistant Board. Master’s degree or certificate from a specialized medical training program associated with a medical school that includes classroom students and clinical experience required.

NURSE PRACTITIONER - Master of Nursing Degree Graduate of an accredited Nurse Practitioner program approved by the California Board of Registered Nursing as defined in Section 1482, Nurse Practice Act, Business and Professional Code required

CERTIFICATIONS, LICENSES, REGISTRATIONS

Current California Registered Nurse License, Current California Licensure and Certificate to practice as a Nurse Practitioner issued by the California State Board and California Furnishing license required.

Current California Licensure and Certificate to practice as a Physician Assistant issued by the California State Board required.

Approval for privileges by Medical Staff (if applicable).

American Heart Association (AHA) Basic Life Support (BLS) for Health Care Providers within 45 days.

TECHNICAL REQUIREMENTS

Knowledge of medical terminology required.
Knowledge of electronic medical record (EMR) required.
Ability to maintain effective and organized systems to ensure timely patient flow required
Experience working with EPIC electronic medical record preferred.
Bilingual in English/Spanish preferred.

YEARS OF RELATED WORK EXPERIENCE

Two (2) years of PA or NP experience or two years of experience specific to the practice specialty of the assigned clinic and/or in an inpatient setting required.
Experience in the area of the clinic specialty and in an outpatient and/or inpatient setting preferred.

KNOWLEDGE, SKILLS, and ABILITIES

Exceptional Independent and collaborative critical thinking.
Exceptional customer service and phone etiquette.
Effective verbal and written communication. Spanish-speaking preferred.
Skilled in correct grammar, spelling, and punctuation.

Pay for non-physician positions is determined based on related years of experience and internal equity.
