Full Job Description

For 30 years, School Health Clinics of Santa Clara County has offered low-cost, high-quality medical services to low income families. Our mission is to provide high quality, affordable primary medical care for the low income community.Research has shown that providing low-income and minority students with healthcare and health education give kids a chance to stay in school and perform better academically. Although we began as pediatric clinics, we now extend our services to adults. No one is ever turned away due to their inability to pay. All five clinics are located on school grounds in Santa Clara County, right in the neighborhoods where our patients live. Our staff is bilingual and multicultural so our patients can easily understand the medical advice that is given and are not intimidated because of a language barrier.

As a team member in the Patient Medical Home, the Licensed Clinical Social Worker collaborates with clinic management, nursing, and clinician staff to maintain high standards of patient care with the patient at the center. The Licensed Clinical Social Worker independently provides social work evaluation and treatment services to at-risk client’s and families with psychosocial needs as needed in general pediatric, adolescent and family medicine clinics; takes a leadership role in coordinating services with the interdisciplinary team and community agencies to ensure appropriate client care; advocates for and provides quality health care services to all persons in a culturally competent manner. The scope of service is directive in nature: to identify and manage clients with medical and mental/ behavioral health needs, provide target treatment, and monitor “at risk,” “high risk,” clients with a focus on the development of problem-solving and self-management skills as well as teaching coping mechanisms. Problems for which clients may be seen include: non-compliance with treatment plans (i.e., diet, exercise, and medications), academic problems (i.e., ADHD), mental health problems related to family disruptions and losses, peer related difficulties, physical /substance abuse and issues surrounding reproductive health and confidentiality.

Sign on bonus-$4000 given after 90 days and $4000 after 180 days.

Responsibilities and Duties

Performs a psychosocial needs assessment of clients and families.
Provide counseling and intervention to pediatric/adolescent clients; conducts individual therapy.
Provide early detection of “at risk” patients, with the aim of preventing further psychological or physical deterioration.
Participates in treatment team, staff, and professional meetings to discuss client progress.
Develops implements and evaluates service plans based on identified needs.
Collaborates with client, family, clinic team members, school staff, primary care provider and community service organizations; maintaining patient confidentiality to ensure service plan and goals are integrated with needs.
Assists the organization to develop resources, communication, and impact in areas pertaining to mental health.
Assist with coordination of grant programs with manager.
Program activities also include identifying / providing in-house training and parenting education.
Assist with data collection to meet funding source requirement.
8-10 patients per day
Comply with agency, program, and professional policies and procedures.
Attend meetings and training's as well as represent the agency, as assigned by the supervisor and as required by the programs.
May perform other related duties.

Qualifications and Skills

Licensure by the State of California as a Licensed Clinical Social Worker or LMFT with pediatric/adolescent client experience.
Spanish speaking preferred
Masters Degree in Social Work from an accredited school.
A minimum of two years work experience in a health care agency or clinical setting, including case management, outreach, and education providing mental health services, preferred.

MINIMUM KNOWLEDGE OF AND ABILITY TO:

Professional verbal and written communication skills, including facilitation, presentation, and writing reports; written communication to be legible, accurate, timely, and reflect established clinical practice.
Critical thinking ability to effectively manage and plan client care.
Experience working with target populations, including people of diverse cultural, education, socio-economic, and linguistic backgrounds, and knowledge of related issues.
Bilingual in Spanish preferred.
Prior experience providing counseling services to Hispanic adolescents, family and community.
Ability to learn and apply clinical skills with the various cultures of the local community. Demonstrated ability to interpret social, emotional and behavioral problems to develop and implement appropriate treatment plan.
Knowledge of theories and research on personality and intellectual growth and development, individual differences, human motivation, and abnormal behavior.
Solid understanding of the bio-psychosocial development stages and medical conditions, including assessment of psychosocial impact of illness, injury and disability.
Ability to assess, diagnoses, and implement plans for clients.
Knowledge of community resources and mandated reporting laws.
Ability to maintain a high standard of professional and ethical conduct.
Proficient in MS Word, Excel, Outlook, and internet research; working knowledge of computers.
Ability to work independently and as a member of multidisciplinary team, prioritize competing assignments, multi-task, problem solve, exercise good judgment, and perform counseling services within a framework of standard policies and procedures.
Car available for daily use, a valid California driver’s license and car insurance as required by the State of California.
Satisfactory completion of employment screening.
