Full Job Description

Job Summary
We are seeking a dedicated and compassionate Case Manager to support adults with developmental disabilities. The ideal candidate will possess a strong background in case management, social work, or behavioral health, with experience in coordinating services and advocating for the needs of our supported individuals. As a Case Manager, you will play a vital role in assessing client needs, developing individualized care plans, and ensuring access to appropriate resources and services to promote independence and well-being.

Duties

Conduct comprehensive assessments of supported individual’s medical, psychological, and social needs to create personalized care plans.
Coordinate services across healthcare providers, community agencies, and support networks to ensure seamless care delivery.
Facilitate intake processes and manage documentation pertaining to supported individuals, in compliance with HIPAA regulations and Regional Center standards.
Monitor supported individuals progress through regular follow-ups and adjust care plans accordingly to meet evolving needs.
Maintain detailed case records and ensure all documentation aligns with Regional Center and organizational policies.
Provide supervision of caregiving staff and ensure they are properly trained at providing a high-standard level of care for our supported individuals.
Work closely and communicate often with Service Coordinators from Regional Center.
Provide crisis intervention and emergency on-call support.

Requirements

At least one year experience in working with adults with developmental disabilities and familiar with the operations of case management.
Familiarity with medical terminology, physiology, diagnostic evaluation, and patient assessment techniques.
Experience working with diverse populations including adults with developmental disabilities or behavioral health conditions.
Strong understanding of HIPAA compliance, records management, and confidentiality.
Excellent communication skills for effective client services, care coordination, and interdisciplinary collaboration.
Background in social work or mental health counseling is highly desirable.
Ability to navigate managed care systems and coordinate services.
Strong conflict resolution skills.
Must be detail oriented, patient, nurturing, possess strong communication skills, and have a passion to help others.
