Overview

We are seeking a dedicated VA Case Manager specializing in Emergency Housing to join our dynamic social services team. In this vital role, you will be at the forefront of supporting veterans experiencing homelessness and housing instability, providing comprehensive case management and connecting clients to essential resources. Your energetic approach and compassionate communication will empower high-risk populations to achieve stability and independence. This position offers an opportunity to make a meaningful impact within diverse communities, ensuring equitable access to safe, affordable housing options.

Responsibilities

Case Management and Residential Care

In a culturally competent manner, provide services to all residents without regard to race, color, creed, national origin, marital status, sexual orientation, age, sex, religion, handicap and all other protected groups.
Assist with planning and implementing individuals’ treatment plans in conjunction with other staff, family and providers.
Assist residents with daily living activities including cooking, cleaning, showering, hygiene and grooming, and other personal care assistance.
Safely assist residents with self-administration of prescription and non-prescription medications in accordance with physicians’ orders.
Abide by the principles of good nutrition, food preparation, storage, menu planning, cooking, clean-up, food safety and food service principles.
Be knowledgeable on housekeeping and sanitation principles. Perform housekeeping duties as necessary. Assist residents in maintaining cleanliness of living areas, including resident rooms and community areas. Assist resident’s to maintain outside lawn and patio areas. Ensure that Residents perform their own laundry.
Monitor and report changes in residents’ physical or mental health status to the resident’s primary care providers and facility staff.
Assist in identifying and accessing medical, educational, social, prevocational, rehabilitative or other community and VA services for residents, specific to resident needs, goals, service plans and housing.
Provide residents with counseling, training and assistance with daily living skills.
Initiate preventative measures and create intervention methods for residents or conditions that pose a threat of danger or crisis.
Monitor and report residents’ progress and outcome data.
Report to Manager any substance use.
Track and assists with scheduling residents’ dental and physician’s exam including TB tests, if applicable.
Assist residents as necessary to attend appointments, outings, etc.
Inform individuals of personal rights and responsibilities, as well as agency confidentiality and grievance procedures. Ensures that services are compatible and coordinated with clients’ freedom of choice.
Maintain cooperative and professional working relationships and communications with residents, co-workers, family members and co-workers.
Give input to management on improving the quality of programs and assists in monitoring program effectiveness and conduct resident satisfaction surveys.
Report suspected abuse, theft and/or loss at the facility to Management.
Other duties and responsibilities as assigned.

Structured Groups

Provide Therapeutic Services including: therapeutic, rehabilitative and recovery services, group activities, VA coordination, clinical case management in collaboration with VA, independent living skills, housing search, outreach and discharge planning per VA contract. 2. Develop monthly group and living skills schedule consistent with needs of the facility. 3. Adhere to monthly schedule of planned groups. C. Reporting and Documentation
Report any abusive or disruptive behavior to the Manager and team.
Responsible and mandated to report any “suspected abuse of dependent adults and elders” directly to law enforcement and Adult Protective Services and any other agency as appropriate. Mandated to comply with suspected abuse reporting policies and procedures
Monitor residents’ progress and participation toward service plan goals, collect and report outcome data. Document coordination in progress notes and complete all documentation (e.g. service plans and progress notes) within required timelines.
Keep clear, accurate, and professional records in resident’s files, staff logs, and other required communications.
Handle all communication (written, verbal, and in-person) in a professional manner.

Experience

Education and/or Experience

High School diploma or GED; two years relevant experience may be required; or equivalent combination of education and experience. B.A. or B.S. in social work or a related field; or two years experience in related field is preferred. Company may waive requirement.

Knowledge

Knowledge of requirements for providing services to residents. Knowledge of Veteran services preferred.
Knowledge and conformity to all applicable HIPAA, OSHA, state and local laws, rules and regulations.
Knowledge of community services and resources and how to access them.

C. Language Skills

Read, speak write and understand English well enough to follow and share the regulations, requirements and communications for providing services to residents.
Able to effectively present information and respond to questions from managers, residents, customers, vendors and the general public with awareness of multi-cultural considerations.
Competency in crisis intervention strategies, crisis management protocols, and social services coordination to support vulnerable populations.

A. Additional Requirements

Must be at least 18 years of age.
If required to drive, must be 21 and accepted by the Company’s vehicle insurance.
Cleared by criminal background check prior to commencing work at the facility.
Maintain a valid First-Aid certification.
May be required to serve on company committees.
Able to answer telephone professionally, understandably, and with proper etiquette.
Attend mandated meetings and trainings; fulfill all coursework and training hours required.
Complete coursework, training hours and pass the Medication Policies and Procedure Examination.

Physical Demands: Must be met to successfully perform the essential job functions.

Regularly required to talk, hear, and sit, stand for extended periods of time.
Frequently required to cook and clean, reach with hands and arms, stand, walk, stoop, lift up to 25 pounds, and may occasionally be required to kneel, crouch or crawl.

C. Work Environments: Employee may encounter while performing essential job functions

Noise level is usually low to moderate.
Possible contact with hazardous and caustic household cleaning chemicals.
The actions of residents may be unpredictable. Staff may be verbally assaulted and at times may be physically injured.
