Summerfield Healthcare is the preferred provider of Post-Acute Rehab services in Santa Rosa, CA and we are seeking a full time Occupational Therapist or COTA to join our team. We have a solid reputation in the community with a long history of success and we need the best of the best to join us on our pursuit of excellence, We are proud of our spotlessly clean facility, well equipped therapy gym and our strong reputation for quality of care. This busy skilled nursing setting offers a stable in-house therapy department where there is ample opportunity for you to create and implement the programs that will best serve your patients. We have an active therapy intern program, experienced and cohesive therapy team, and excellent staff retention. As a therapist at Summerfield, you will gain experience with a diverse and interesting patient population.

Although prior therapy experience is preferred, we place a high value on intelligence, drive and willingness to learn and grow, making this an excellent opportunity for an exceptional new graduate who is willing to work hard and make him or herself an indispensable member of our team. This is not a sleepy, boring, and old nursing home that some might envision. We are a fast paced Post-Acute Rehab facility with a robust therapy program that caters to patients needing strengthening and reconditioning before transitioning home after an inpatient hospital stay. As a 56 bed facility our smaller size provides a more quaint and "home-like" family environment that allows for a more personalized approach to the delivery of patient care—an environment we feel plays a major role in the healing and speedy recovery of our patients. It also provides a unique opportunity for our staff to build a rapport with our patients and other staff alike that they otherwise would struggle to have in a larger facility. Recent renovations help us stand out amongst our competitors and we expect that you’ll notice right away when entering how clean and inviting our facility is.

THE CASELOAD

Summerfield has worked incredibly hard over the past couple of years to influence our reputation in the community and become the rehab facility of choice. This concentrated effort has enabled us to achieve a preferred partner status with 3 of our local acute care hospital providers which has led to a steady increase in hospital-directed referrals. This has all but eliminated the ebbs and flows that most facilities experience with their rehab caseload and you can look forward to consistency with your hours here at Summerfield.

We have recently upgraded some of our technology and firmly believe that the right technology allows for greater efficiency and enhances the employee and patient experience and ultimately leads to better quality care for our patients. We even use a HIPPA compliant text messaging application that has streamlined our communication with physicians and management.

5 Star Rating by CMS - 1 of only a few facilities in our market to be rated 5-star overall but also with a 5 star rating in all 3 of the components (Health Inspection, Staffing, and Quality Measures).
1 Deficiency Health Inspection Survey – 1 of only a few facilities to have achieved such a result (CA average is 9.6 deficiencies).
Recognized in the top 10 out of 1,153 facilities for "Customer Satisfaction" by National Research Corporation (MyInnerview) who conducted resident satisfaction surveys on our behalf.

In addition, we have been awarded the prestigious Ensign Flag Award and are determined to win it again this year. The award includes a $50,000 reward, half of which is distributed amongst our staff as a bonus, the other half goes towards equipment upgrades, facility renovations, etc. Furthermore, this award differentiates us from our competitors and demonstrates the uniqueness of the experience we provide for our patients and staff.
