Full Job Description

PACE Medical Director

Job Summary

The Medical Director provides clinical leadership and direction to AgeWell PACE related to the delivery of comprehensive medical care and serves as a primary care provider for PACE participants. The Medical Director ensures the delivery of high-quality, participant-centered health care services across the interdisciplinary team (IDT), including oversight of preventive, acute, chronic, and geriatric care in accordance with PACE regulations and standards of practice. The role supports and directs medically related AgeWell PACE committee work, quality improvement initiatives, and clinical policies to promote optimal health outcomes for participants.

Position Responsibilities: Administrative

Provides leadership and medical expertise in the development of medical policies, procedures and guidelines.
Responsible for the development of AgeWell PACE clinical standards and medical practice guidelines and protocols.
Provides oversight of the QI Plan.
Reviews all quality-of-care issues and oversees the development and implementation of quality-of-care corrective action plans.
Participates in the oversight, training and education of the interdisciplinary team.
Coordinates performance appraisal of the Primary Care Physician and Advanced Practice Providers.
Develops educational and other programs to build the skills of participating providers.
In conjunction with Contract Manager manages all communication with the provider network.
Represents AgeWell PACE to external agencies, professional groups and regulatory agencies and organizations as required.
Responsible for delivery of care and clinical outcomes.
Provides medical guidance and supervision of medical services activities.
Routine travel when necessary.

Position Responsibilities: Clinical

Perform comprehensive history and physicals on new referrals.
Performs interval history and physicals on established participants, completed every 180 days or as indicated.
Evaluates and treats participants with episodic illnesses.
Refers participants to Medical Specialist(s) as indicated.
Admits participants to the hospital when indicated: (a) Provides primary care (attending) responsibilities for management; (b) Provides updates to the IDT on a regular basis; (c) Judiciously utilizes specialty consultants; (d) Responsible for coordinating discharge planning with the IDT.
Manages all care of participants who reside in nursing homes: (a) Provides regular visits; (b) Performs telephone contacts with nursing home staff as required; (c) Admits nursing home participants to hospital when necessary.
Actively participates in family conferences regarding care planning and implementation.
Coordinates 24- hour care delivery.
Documents participant changes appropriately in the medical records and communicates participant changes to IDT in a timely manner.
Participates in on-call coverage rotation.
Participates as a member of the IDT. Attends staff meetings and take part in participant care planning.

Meet a standardized set of competencies for the specific position description established by AgeWell PACE and approved by regulatory agencies before working independently.
Be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant contact.
Pass a criminal background which will verify that the candidate is: not excluded from participation in Medicare and Medicaid programs; has not been convicted of criminal offenses related to their involvement in Medicaid, Medicare, other health insurance or health care programs, or social services programs under title XX of the Act; and have not been convicted of physical, sexual, drug or alcohol abuse.

Knowledge of

Geriatric medicine; special needs of complex geriatric patient.
Culture and needs of the socially and ethnically diverse population served by AgeWell PACE.
Principles and practices of health care, health care systems, and managed care.
Community-based organizations, agencies, and community services.
Team and value-based care.
Regulatory policies, rules and regulations.

Experience & Education

M.D. or DO with current state of California license.
DEA registration and the ability to obtain and maintain staff privileges at AgeWell PACE contracted agencies.
Board certified in Internal Medicine or Family Practice with advanced certification in geriatrics, preferred.
Minimum 3 years’ experience in a lead administrative role and 1 year of primary care experience with a frail, elderly or long-term care population.
Experience working in a managed care environment and working with peers and other health providers to resolve utilization, quality management, performance improvement, pharmacy and therapeutics, peer review, credentialing, and physician leadership issues.

Physical demands: Employee must be able to sit for extended periods of time. Employee will also be actively present throughout the PACE center, engaged with employees, participants and with staff. Employee is required to use hands and fingers, especially for typing on the computer and using the mouse. Employee must be able to talk and hear, particularly for daily interactions with staff and elderly participants.
Work Environment: Employee must be able to work in a busy and demanding clinic environment with moderate to loud noise levels.
