Social Services Director

Skilled Nursing & Rehabilitation Center | Florida

Are you a compassionate, organized, and resident-focused Social Services professional looking for an opportunity to make a lasting impact? Our skilled nursing and rehabilitation center is seeking a Social Services Director who is passionate about advocating for residents, supporting families, and enhancing quality of life through exceptional psychosocial care.

What You'll Do

As our Social Services Director, you will play a vital role in ensuring residents receive the emotional, social, and psychological support they need throughout their stay.

Responsibilities include:

Direct and oversee the Social Services Department in accordance with federal, state, and facility regulations.
Complete comprehensive psychosocial assessments and develop individualized care plans.
Participate in interdisciplinary care plan meetings and collaborate with the clinical team.
Assist residents and families with admissions, discharge planning, transfers, and transition of care.
Provide counseling, crisis intervention, emotional support, and advocacy for residents and their families.
Coordinate community resources and referrals to support resident needs.
Assist residents with advance directives, resident rights, and grievance resolution.
Maintain accurate, timely, and compliant documentation.
Promote resident satisfaction, dignity, and quality of life.
Ensure compliance with CMS, AHCA, and all applicable regulatory requirements.
Participate in Quality Assurance and Performance Improvement (QAPI) initiatives.

Qualifications

Bachelor's degree in Social Work, Human Services, Psychology, Sociology, or a related field required.
Florida Licensed Clinical Social Worker (LCSW) preferred but not required unless required by facility needs.
Previous experience in a skilled nursing facility, rehabilitation center, long-term care, or healthcare setting preferred.
Knowledge of Medicare, Medicaid, CMS regulations, and resident rights.
Strong communication, organizational, conflict resolution, and problem-solving skills.
Ability to build trusting relationships with residents, families, and interdisciplinary team members.
Experience with electronic medical records (EMR) preferred.
