Full Job Description

Now Hiring: Full-Time Program Coordinator (245D Residential Services)
Location: Saint Cloud, MN

Schedule: Full-Time, primarily Monday-Friday with flexibility to respond to program needs. Participation in an on-call rotation is required and may occasionally require evening, weekend, or holiday response to support program operations.

Anthurium Human Services is seeking a compassionate, organized, and dependable Program Coordinator to join our team! If you're passionate about supporting individuals with disabilities and enjoy building positive relationships with both individuals served and your team, we'd love to hear from you.

This position oversees the direct day-to-day operations of one or more 245D residential programs, working closely with the Director and organizational leadership to ensure high-quality, person-centered services. This role serves as the primary on-site leader by supporting individuals served, coaching and supporting Direct Support Professionals (DSPs), coordinating daily operations, and helping ensure services are delivered safely and in compliance with Minnesota Rule 245D requirements.

Qualifications

Leadership or supervisory experience in the human services field is preferred
Strong communication, organization, and documentation skills
Ability to pass a DHS background study
Valid driver's license, reliable transportation, and proof of auto insurance
Ability to safely perform the essential physical function of the position, including assisting individuals served and responding to emergency situations when necessary

Key Responsibilities

Oversee the daily operations of assigned residential 245D programs, ensuring safe, consistent, and person-centered services.
Provide on-site leadership, coaching, mentoring, and ongoing support to Direct Support Professionals (DSPs).
Facilitate on-site training and shadow shifts for new employees to promote confidence and success in their roles.
Assist with implementation of individual support plans, outcomes, and positive support strategies.
Help maintain program structure through daily schedules, activities, and household routines.
Monitor documentation and service delivery to support compliance with Minnesota Rule 245D.
Foster a positive, respectful, and team-oriented work environment through coaching, communication, and collaboration.
Communicate effectively with individuals served, families, guardians, case managers, healthcare providers, and other members of each individual's support team.
Participate in an on-call rotation and provide leadership during emergencies, staffing challenges, and other program needs.
Use sound judgment and problem-solving skills to support daily program operations and address challenges as they arise.
