Union Mission is seeking an experienced and dedicated Case Manager to join our team. In this role, you will work directly with individuals experiencing housing instability, helping them identify needs, access community resources, develop goals, and move toward greater independence.

What You’ll Do

As a Case Manager, you will serve as an advocate, resource, and partner for clients needing specialized housing and supportive services. You will conduct comprehensive assessments, coordinate services, connect clients with community resources, and provide individualized, client-driven case management.

Your primary responsibilities will include:

Managing a caseload of clients participating in Union Mission’s housing programs
Completing comprehensive client assessments and intake documentation
Partnering with clients to establish realistic, measurable goals that support housing stability and self-sufficiency
Connecting clients with appropriate social services, healthcare providers, and community resources
Responding appropriately to client crises using trauma-informed and client-centered practices
Assisting clients referred by other providers with identified social-service needs
Building and maintaining positive relationships with clients, service providers, and community partners
Conducting periodic community meetings with housing-program clients
Planning and engaging clients in meaningful community activities
Documenting client services and progress accurately and promptly in HMIS
Preparing required monthly reports and maintaining complete, current client files
Protecting confidential client and organizational information
Assisting with the daily operations of the program and office
Performing other duties as assigned by the Program Manager

What You’ll Bring

Required Qualifications

Bachelor’s degree in social work, human services, or a closely related field
At least five years of experience in a social-work or human-services setting
Demonstrated success providing effective case-management services
Experience with crisis management, trauma-informed care, and professional documentation
Strong communication, relationship-building, and problem-solving skills
Ability to work effectively with individuals from diverse backgrounds and those experiencing complex challenges
Compassionate, empathetic, client-focused, and results-oriented approach
Ability to establish and maintain productive relationships with clients and community partners
Proficiency in Microsoft Office applications
Ability to learn and effectively use the required HMIS database
Valid driver’s license

Preferred Qualifications

Master’s degree in social work or a related human-services field
Knowledge of substance-use disorders, mental health, and HIV-related services
Relevant lived experience that supports an authentic understanding of and connection with the clients served
