Job Title: Case Manager – CSAH Housing Communities

Reports To: Assistant director of Housing and Coordinated Entry (CE)
Location: 65 Dundee, Savannah, Ga.
Job Type: Full-Time

Position Summary

CSAH is seeking a compassionate, organized, and equity-driven Case Manager to support residents living in our innovative tiny home and cottage-style housing communities. These communities serve individuals and families exiting homelessness, offering both bridge housing and permanent supportive housing (PSH).

The Case Manager will work closely with the Assistant Director of Housing and Coordinated Entry (CE) and Housing Manager to provide individualized support, promote housing stability, and ensure residents have access to the services and resources they need to thrive. This role is essential in fostering a safe, inclusive, and trauma-informed environment where every resident is treated with dignity and respect.

Key Responsibilities

Resident Support & Case Management

Provide strengths-based, person-centered case management to residents, including goal setting, service planning, and regular check-ins.
Assist residents in navigating housing transitions, including move-ins, lease renewals, and exits.
Support residents in accessing benefits, employment, healthcare, mental health services, and other community resources.
Advocate for residents’ needs while promoting self-determination and independence.

Collaboration & Coordination

Work collaboratively with the Housing Manager to address resident concerns, lease compliance, and community dynamics.
Coordinate with external service providers, healthcare professionals, and community partners to ensure wraparound support.
Participate in multidisciplinary team meetings and case conferences.

Documentation & Compliance

Maintain accurate, timely, and confidential case notes, service plans, and progress reports.
Ensure compliance with HUD, PSH, and bridge housing program requirements.
Assist with data collection and reporting for grants, audits, and internal evaluation.

Community Engagement & Equity

Foster a welcoming, inclusive, and trauma-informed community culture.
Uphold CSAH’s commitment to racial equity, accessibility, and housing justice in all interactions.
Incorporate feedback from residents with lived experience into service planning and program improvement.

Professionalism & Confidentiality

Maintain the highest standards of confidentiality, professionalism, and ethical conduct.
Represent CSAH with integrity in all interactions with residents, staff, and external partners.
Protect the organization’s reputation and mission through respectful, mission-aligned service delivery.

Qualifications

Bachelor’s degree in Social Work, Human Services, Psychology, or related field (or equivalent experience).
Minimum 2–4 years of experience in case management, supportive housing, or homelessness services.
Knowledge of trauma-informed care, harm reduction, and housing-first principles.
Strong interpersonal, organizational, and crisis intervention skills.
Ability to work independently and collaboratively in a fast-paced, community-based setting.
Commitment to equity, inclusion, and culturally responsive service delivery.
Proficiency in documentation and case management systems.

Preferred Qualifications

Experience working in supportive housing or with tiny home/cottage communities.
Familiarity with HUD regulations and PSH/bridge housing models.
Bilingual or multilingual abilities.
Lived experience of homelessness or housing instability is highly valued.
