Full Job Description

Definition

Under direct supervision from the Behavioral Health Services Manager, provides case management services to either children/families or the adult clients of the Division of Behavioral Health Services programs. Incumbent ensures that quality, effective and efficient services are provided to clients and that the clients are receiving services to meet their basic needs of becoming self-sufficient. Incumbent assists clinical staff in the implementation of specific treatment plans and coordinates with other service providers to ensure the necessary psychological, social and economic services/entitlements are acquired and provided. This job class is treated as FLSA Non-Exempt.

Essential Functions: Essential functions may vary among positions and may include the following tasks, knowledge, skills, abilities, and other characteristics. This list of tasks is ILLUSTRATIVE ONLY and is not intended to be a comprehensive listing of tasks performed by all positions in this classification.

Examples of Tasks

1. Recruits, identifies, and interviews eligible clients in accord with program criteria. Evaluates social, mental health/substance abuse, behavioral and financial service needs, and verifies information provided by clients.

2. Provides case management services for eligible clients by developing cost effective and clinically appropriate service plans. Arranges for provision of or referral to necessary services and monitors service plans. Develops discharge plan and documents necessary information in clients' records. Coordinates case management services with respective clinical staff.

3. Confers with representatives from public and private agencies and organizations such as, but not limited to, Social Security, State/Tribal Welfare Offices, AHCCCS, Legal Aide, Psychiatric hospitals, Behavioral Health Agencies, and Residential Treatment Facilities to assist clients in applying for and obtaining appropriate needed services.

4. Ensures clients' compliance through documentation of participation and progress on treatment plan. Assists with client support services, such as arrangements for childcare and transportation to ensure compliance and progress.

5. Maintains statistical data on clients to meet program's reporting requirements. Prepares reports, case histories and case studies as required. Maintains accurate database for easy retrieval and reference purposes. Prepares periodic reports from records, as required. Maintains current case files. Ensures accurate record keeping, using computerized database. Records information to ensure maximum confidentiality. Establishes and maintains client records in accordance with program policy and procedures.

6. Participates in weekly case staffing or staff meetings. Discusses issues or cases that require additional input and planning. Provides alternative solutions to issues to ensure the greatest benefit for clients in accomplishing the client goals. Provides updates on the status of clients in long-term residential treatment to clinical staff.

7. Serves as communicator and facilitator between individuals, families, clinic staff, other community health personnel, contract providers and state agencies to ensure continuity of care between providers. Monitors client's compliance with prescribed treatment plan, including making reports to physician on psychotropic medical treatments.

8. Provides support and assistance to family members in dealing and coping with client's illness, difficulty, mental health/substance abuse problem, etc.

9. Provides transportation for clients, as needed.

10. Performs other duties as assigned to maintain and enhance program and agency operation.

Knowledge, Skills, Abilities and Other Characteristics:

Knowledge of the culture, customs, traditions, history and government of the Salt River Pima- Maricopa Indian Community.
Knowledge of the needs of families within the SRPMIC.
Knowledge of policies, procedures, rules and guidelines (State and Tribal) for related HHS programs, including procedures on confidentiality of case files.
Knowledge of such programs as, but not limited to: tribal mental health ordinance, involuntary commitment, AHCCCS, and Title IXX.
Knowledge of case management techniques and practices as well as crisis intervention methods.
Knowledge, at a working level, of the uses and side effects of psychotropic medications.
Knowledge (basic) understanding of behavioral problems and serious, often chronic, mental illness and substance abuse.

Skill in assessing client needs and identifying resources/services available to meet needs.
Skill in interviewing clients under stressful situations.
Skill in establishing and maintaining effective working relationships with the clients, SRPMIC staff, and representatives of resource agencies.
Skill in using the personal computer and related software, in order to fulfill job requirements.

Ability to communicate effectively, both orally and in writing.
Ability to establish and maintain accurate record keeping systems and procedures, including computerized record keeping.
Ability to working evenings, weekends and to travel in state, as needed.
Ability to make home visits and provide alternative solutions for family services needed.
Ability to drive various SRPMIC vehicle(s).
Ability to work with a variety of staff members in a team approach to service delivery.
Ability to operate a variety of standard office equipment including photocopiers, FAX machines, typewriters and personal computers.

Minimum Qualifications

Education & Experience: Bachelor's degree in Counseling, Social Work, Psychology or closely related area and experience equivalent to three (3) years full-time in social services field or with community outreach work. In lieu of the Bachelor's degree, candidates with an Associate's degree in a related field and experience equivalent to five (5) years full-time professional level experience in social services, community outreach or a rehabilitation treatment setting will be acceptable.

Equivalency: Any equivalent combination of experience that will allow the applicant to satisfactorily perform the duties of the job may be considered when filling the position.

Underfill Eligibility: An enrolled Community Member whom closely qualifies for the minimum qualifications for a position may be considered for employment under SRPMIC Policy 2-19, Underfill

Special Requirements

May be required to work beyond normal work hours including nights, weekends and holidays.
Must have current certification in First Aid and CPR prior to working with patients.
Due to the confidential and sensitive nature of this position, successful completion of an extensive background investigation is required.
May be required to travel
Must possess and maintain a Valid Arizona Driver’s License.
Employees in, and applicants applying for, jobs providing direct services to children are subject to the “Community Code of Ordinances”, Chapter 11, “Minors”, Article X. “Investigation of Persons Working With Children”. New hires in clinical roles must present valid BLS certification (Healthcare Provider BLS for clinicians) prior to the first day of patient care.
Exception – For all Journey to Recovery direct care employees, valid CPR/BLS certification and First Aid certification must be provided on the first day of employment or within 7 days of hire.
Ensures all patient information is kept confidential and complies with Health Insurance Portability and Accountability Act (HIPAA) regulations and SRPMIC HHS policies and procedures.

Prior to hire as an employee, applicants will be subject to drug and alcohol testing. Will be required to pass a pre-employment background/fingerprint check.
