We are currently recruiting an experienced Director of Wellness who has a passion for working with seniors and shares our commitment to providing quality care for seniors and their families. The Pines Senior Living is a personal care and memory care facility located on Morgan Highway in Clarks Summit, PA.
Responsibilities:
*Provide leadership for resident care in our community by managing, directing and monitoring the care team staff, as applicable by State law, to promote the health and wellness of the resident population
*Provide clinical care through the direct application of the nursing process; perform and document resident assessments, complete individual care plans, provide or delegate hands on clinical care as indicated by the plan of care and evaluate resident outcomes.
*Provide oversight of the community medication management program to promote resident safety in the medication process
*Collaborate with physicians, pharmacists and other clinical providers to coordinate care and services for the residents in the community.
*Provide clinical training and education as needed, to nurses, medication technicians, and others who provide resident care. Perform skill evaluations to assure clinical capability of the care team members.
*Track, trend and report clinical quality data to identify risk and lead improvement initiatives to reduce risk and improve resident outcomes
*Recruit, hire, and train clinical team members and be responsible for staff management, scheduling, evaluation and engagement.
*Collaborate to assess and determine residents’ appropriateness for move-in to the community
*Assure compliance with all Federal, State, Board of Nursing and other applicable regulations
Qualifications:
- Current LPN license in good standing. Ability to pass a criminal background check.
- A minimum of one year of experience working with residents in a long-term care setting and 3 years in nursing
- Strong leadership ability
- Ability to tactfully deal with residents, family members and other care providers
- Provide clinical care through the direct application of nursing process; perform and document resident assessments, complete Individualized Care Plans, provide or delegate hands on clinical care
- Knowledge of applied nursing practices, techniques, and methods in accordance with federal, state/province and board of nursing requirements.
- Knowledge of infection control practices and prevention of disease transmission.
- Ability to delegate assignments to the appropriate individuals based on their demonstrated skill capability and in compliance with all regulations.
- Proven ability to handle multiple priorities, organize efficiently and manage time effectively.
- Demonstrated critical thinking, clinical judgement, and decision-making skills.
- Computer proficiency with electronic medical records, Microsoft Office suite and the ability to learn new applications.
- Ability to work weekends, evenings & flexible hours as needed for resident care/services, including 24/7 on call responsibility.
