The Opportunity

This is a Admissions position for a skilled nursing facility.
Primary duties include:
- Follows a proven referral response protocol.
- Develops and maintains business relationships and census development plans.
- Assesses patient referrals to determine admission eligibility.
- Directly works with hospital case management, facility clinicians, and financial staff to facilitate patient admissions.
- Conducts primary and coinsurance verifications.
- Coordinates admission arrangements including transportation, medical equipment needs, insurance authorizations, and room readiness checks.
- Completes admissions paperwork completely with the patients and/or families.
- Plans and coordinates community marketing events.
- Develops contacts and builds new primary, secondary, and tertiary referral sources in the market.
- Must have a team success mindset and ability to work and communicate with others professionally.
- Completes daily reporting and meeting attendance.
- Maintains compliance with state and federal regulatory requirements.
- Must be able to set and achieve goals and reach target admissions metrics.
Skills/ Requirements
- 3 - 5+ years healthcare experience preferred.
- Licensed nurse preferred.
- MUST have current knowledge of Medicaid, Medicare, and Commercial insurance verification systems.
- Knowledge of LTC/SNF clinical operations and procedures.
- Demonstrates an understanding of patient mix, regulatory requirements, reimbursements, etc. that impact overall nursing home operations.
- The ability to multi-task and identify/interact with various referral sources.
- The ability to communicate in English via phone, in writing and verbally in conversation with different levels of staff, patients, families, and other external contacts.
- Must demonstrate a work history of being a self-starter, working independently with minimum daily direction, and self-motivation with goals.
