Develops, implements, evaluates, and revises clinic-specific objectives, priorities, policies, procedures, QI activities, and standards of care and practice, which are consistent with those for the overall clinical support programs in collaboration with the team.
Participates in the overall risk management activities to ensure timely follow-up and action plan, with reports sent to the supervisor.
Maintains professional knowledge and proficiency in nursing through continuing education, staff meetings, workshops, internal training programs, and serves on committees as needed.
Participates in clinical and peer review to evaluate, modify, and improve current practice.
Provides and performs comprehensive professional nursing care duties for patients with a wide variety of injuries and illnesses.
Studies clinical support staff operations to identify areas to improve care, meet patient satisfaction, and efficient resource utilization; recommends actions and considerations to the management.

Utilizes and reviews patient charts and records for the identification and evaluation of clinical, social, and logistical problems, thus improving the thoroughness and quality of care delivered.
Assures the provision of outpatient services and functions by assisting and acting as back-up to other outpatient staff as needed, sharing extra duties and overtime as assigned.
Acts as a patient advocate by striving for respectful, thorough, continuous, and quality care and adherence to the guidelines found in the Patient’s Bill of Rights.
Educates patients and families in a wide variety of health care subjects such as disease prevention, lifestyle issues, basic health care maintenance, and self-administration of medications.
Triages all patients for basic vital signs, main complaints, and purpose of visit, and reviews charts for recent visits, chronic problems, recent and outstanding laboratory studies, and other information that would facilitate prompt and complete care.
Provides telephone triage for patients requesting information or advice and assists with and encourages appointments when appropriate.
Works with medical providers to assure the smooth flow of patients through examination rooms, laboratory, pharmacy, medical records, and any other activities involved in the outpatient visit. This would include assistance with arranging referrals and follow-ups, locating test results, making phone calls, and handling clinical patient care correspondence as needed.
Provides post-care evaluation and patient education before discharge of patients as indicated.
Performs or directs as appropriate patient follow-up, including notifying patients of laboratory results, medication refills, sending, scheduling, and tracking referral appointments, maintaining charts and records, and responding to patient telephone calls.
Assists providers and the clinical support team in managing patients. Tracks and recalls patients for chronic care follow-up and preventive care visits when appropriate.
Works closely with our tribal healthcare partners across the state to coordinate care across the continuum for our patients.
Assures that the examination rooms are supplied with the necessary equipment and supplies to facilitate smooth operation.
As requested, participates in and/or provides health education opportunities for communities in conjunction with other Chugachmiut staff. Activities may include providing health education and screening services at schools, health events, and other community events.
Works in coordination with management and the healthcare team to develop and implement a quality assurance program, providing and receiving consistent feedback on the quality of care being delivered to ensure regulatory standards are met as they apply to the division.
Makes meaningful improvement to services, programs, and processes and/or organizational effectiveness that create new value for patients and employees, such as assisting in monitoring the quality of care and assisting in completing chart reviews regularly.

Essential Duties and Responsibilities (Vaccine Coordinator):
Reviews and maintains the VFC Provider Agreement between Chugachmiut and the State of Alaska Immunization Program.
Completes required VFC Provider Staff Training and ensures Back-up Vaccine Coordinators complete the required training.
Updates and submits a modified VFC Provider agreement after any staff or facility changes.
Manages staff’s access to VacTrAk and provides quarterly vaccines due reports to clinics.
Manages and maintains a Vaccine Emergency Plan, reviews and trains new staff and provides refresher training on an annual basis.
Ensures clinical staff are trained on all policies and procedures regarding vaccines.
Works with clinical staff to monitor the operation of storage units to ensure they are working correctly.
Works with clinical staff to ensure refrigerator and freezer units maintain stable temperatures for vaccine storage.
Works with clinical staff to monitor temperature data loggers to ensure they are working correctly and calibration certificates are not expired.
Collects and reviews monthly temperature logs for all vaccine storage units to ensure current, minimum, and maximum temperatures are recorded once a day by staff.
Closely monitors vaccine inventory across the region and orders vaccines as needed.
Works with clinical staff to properly receive and store vaccine orders.
Coordinates vaccine transportation to remote villages when additional inventory is needed and follows the Alaska Immunization Program’s Transport Methods.
Completes VacTrAK Monthly Inventory Assessment for region clinics.
Reviews temperature monitoring devices weekly to ensure no temperature excursions have occurred.
In the event of a temperature excursion:
Review temperature data and contact vaccine manufacturers to obtain stability data on the viability of the vaccines.
Complete temperature excursion report and a Vaccine Return in VacTrAK and submit to the State of Alaska Immunization Program.
State vaccines found to no longer be viable are packaged up and returned to the State of Alaska Immunization Program.
Works with clinical staff to ensure they are rotating stock so the vaccine with the earliest expiration dates is used first to avoid vaccine waste due to expiring.
Works with clinical staff to ensure expired vaccines are removed from storage units to avoid unintentional use of expired vaccines.
Works with clinical staff to return expired State vaccines back to the State of Alaska Immunization Program.
Is available to assist staff with implementing the Vaccine Emergency Plan when needed during a power outage or equipment failure. Establishes backup coordinators to serve as a resource for staff when needed.
