Full Job Description

Serving Sheboygan, Manitowoc, Kewaunee, and Door counties

Overview

Lakeshore CAP is proud of its supportive work culture. We encourage creativity and individual style when providing services to our community.

From homelessness to supportive housing, Lakeshore CAP provides housing programs, support, and other solutions to address the housing needs in our community.

The Supportive Housing Case Manager works with those that are homeless or at imminent risk of homelessness, provides referrals, and offers supportive services for individuals and families in Sheboygan, Manitowoc, Kewaunee, and Door counties.

Responsibilities

Performs intake, confirms eligibility, and provides case management services using a trauma informed approach.
Assists households in developing personalized goal plans, monitors and supports progress of participants while providing individual case management. Refers participants to relevant programs through LCAP and/or partners.
Enters data in agency tracking systems in a timely, accurate way. Runs necessary and applicable reports within data systems.
Takes an interdisciplinary, trauma-informed approach to case management. Seeks out practical, innovative solutions to problems by working with coworkers and other agency partners to address issues of clients and programs.
Is able to represent Supportive Housing programs and Lakeshore CAP to various audiences of professionals and community members.
Participates in the biennial point-in-time count, a nationwide comprehensive census of people experiencing homelessness on a single night. Conducts outreach to those without shelter.
Complies with all confidentiality requirements. Participates in regular supervision, agency staff meetings, assigned trainings, networking meetings and other events as directed.
Maintains positive relationships with clients, coworkers, service providers, volunteers, and community members.

Job Requirements

Must be passionate about Lakeshore CAP’s mission.
BA/BS in a social work related field or equivalent work experience preferred.
Excellent computer skills in a Microsoft Windows environment
Ability to maintain a high level of confidentiality, a professional demeanor to represent the organization
Must be able to pass a criminal background check
Must have a reliable vehicle, valid Wisconsin driver’s license and current personal auto insurance as required by law to conduct client meetings and inspections.
Understands and implements or is willing to learn a Trauma Informed Care approach.
Must possess strong oral and written communication skills, be dependable and adaptable.
