Title: Help at Home Clinical Manager
Location: 211 Smith Street, Sheridan, WY 82801
Reports to: Director of Operations
ESSENTIAL JOB FUNCTIONS
Leadership and Management
- Demonstrates positive leadership by upholding the organization's cultural values, vision, and mission.
- Oversees the day-to-day operations of the Help at Home Services Program, assuring effective and efficient processes are in place to provide service excellence for the people we serve.
- Responsible for the fiscal management of the program.
- Participates in the annual planning and budget development process.
- Conducts Quarterly Quality Assurance meetings and assures follow-up.
- Participates in program development and strategic planning.
- Is a member of the organization’s Risk Management Team.
Client Services and Support
- Provides one-on-one assistance for clients as needed that promote independence, quality of life, and dignity - including but not limited to information, intake, evaluation, plan of care development and implementation when applicable, case supervision when applicable, service termination when applicable, follow-up and quality assurance.
- Provides Help at Home clients with information regarding services to promote independence, quality of life, and dignity.
- Works closely with other program areas and community services to collaborate and problem-solve circumstances as necessary for the effective delivery of services.
- Assists with scheduling Help at Home clients and aides using the organization’s software (Alora).
Compliance and Quality Assurance
- Oversees Help at Home Service Program licensure & compliance.
- Maintains and complies with current state and federal regulations.
- Oversees Wyoming Home Services, Medicaid Community Choices Waiver, VA Home Health Services, Long Term Care Insurance, and Private Pay Services for the organization.
- Conducts supervisory review of Help at Home CNAs, LPNs, and Day Break CNAs.
Human Resources and Training
- Participates in Human Resource Management activities including recruiting, interviewing, hiring, training of new staff, certification, annual evaluations, coaching, and facilitating performance improvement plans as needed.
- Regularly conducts or facilitates training for employees.
Administrative and Operational Tasks
- Develops and maintains accurate and complete records including staff and client.
- Enters billing information and works with Finance to ensure the quality of the billing procedure.
- Manages electronic tablets and software related to Help at Home.
- Works with the Facilities Coordinator to oversee vehicle maintenance and maintain the vehicle schedule to minimize personal mileage for staff, maximize the use of the Help at Home Fleet, and transport clients as needed.
Communication and Collaboration
- Collaborates with all program areas of the organization to ensure the goals and objectives of the organization are met.
- Works cooperatively with other agencies and community professionals.
- Communicates information affecting older adults and other members of the public utilizing the local media and various publications.
- Provides program information for grants and fundraising activities.
Physical Demands
- Frequently communicates and exchanges information with others
- Regularly observes and evaluates people served and staff in Help at Home Services Program
- May be required to sit at a computer for extended periods of time to complete essential job functions.

- Experience in fiscal management
Minimum Qualifications
- Licensed Nurse with current license in the State of Wyoming, with one year of experience in a home health setting required.
- Experience working with older adults or people with disabilities preferred.
- Must have a reliable vehicle, a valid driver’s license, insurance, and a safe driving record.
- Employment is dependent on the ability to pass a Central Registry and Division of Criminal Investigations background check and provide a clean, current Motor Vehicle Department Report.
