Assignment Details
- Position: Speech Language Pathologist, CCC-SLP
- Location: Santa Clara, NM
- Setting: Long-Term Care
- Contract Length: 13 weeks
- Extensions: Multiple extensions possible
- Shift: Days | Schedule: Monday–Friday | 8:00 AM–5:00 PM
- Start Date: ASAP
Requirements
- Active and unrestricted New Mexico SLP license
- Current ASHA Certificate of Clinical Competence (CCC-SLP) preferred
- Previous long-term care or skilled nursing experience preferred
- PointClickCare experience required
- Current CPR/BLS certification, if required by the facility
- Strong clinical, documentation and interdisciplinary communication skills
Key Responsibilities
- Evaluate and treat residents with speech, language, communication and swallowing disorders
- Develop individualized treatment plans based on clinical findings and resident goals
- Complete accurate and timely documentation using PointClickCare
- Collaborate with nursing, therapy, dietary and medical teams
- Educate residents, families and caregivers on treatment strategies
- Monitor progress and adjust plans of care as needed
